About this file
This document is an Attachment D Proposed Work Plan for a Regional Health Equity Coalition (RHEC) grant opportunity with the Oregon Health Authority (OHA) Equity and Inclusion Division. The grant covers an 18-month period from January 2026 through June 30, 2027, focusing on building health equity capacity among underrepresented Communities of Color. The work plan outlines six primary goal areas: creating core operational documents, building coalition structure and governance, strengthening internal organizational capacity, conducting community needs assessments, developing regional strategic plans, and participating in required meetings and reporting.
The grant requires extensive collaboration, documentation, and reporting, with specific deliverables including monthly conference calls, tri-annual work plan status updates, semi-annual and annual reports, expenditure reports, and participation in site visits and gatherings. No disbursements will be provided until budget approval by both the Fiscal Sponsor and OHA. The grantee must develop partner agreements, convene partners at least ten times per year, create a coalition governance charter, conduct community needs assessments, write a regional strategic plan, and provide equity-related training. The project emphasizes cross-sector involvement, engagement with Communities of Color, and capacity building to address Social Determinants of Health and improve health equity.
