About this file
This document is an addendum to a Request for Applications (RFA) issued by the Oregon Health Authority (OHA) for Trauma Hospital Surveyor services. The RFA seeks to contract with Emergency Physicians, Trauma Surgeons, General Surgeons, and Nurses with expertise in Trauma Program Management to conduct Site Reviews and Focused Reviews of Oregon hospitals to assess compliance with the state's trauma system statutes and rules. The addendum extends the application submission period, specifying that applications will be accepted on an ongoing basis through August 31, 2025, or until OHA determines it has received a sufficient number of applications. OHA reserves the right to issue a notice specifying a date after which applications will no longer be accepted.
The contract period is open-ended, with OHA able to issue contracts at any time throughout the submission period. No specific pricing, set-asides, incumbents, funding sources, or other salient financial details are provided in this addendum. The focus is on the administrative details related to the application submission and review process.
