About this file

This document is an addendum to a Request for Applications (RFA) issued by the Oregon Health Authority (OHA) for Equity & Inclusion; Anti-Racism; and Health Equity Services (RFP: S-44300-00003594 OHA #5407). The addendum modifies Section 16 "Closing Date for Submittal of Applications," extending the application period from approximately 90 days to one year. Applications will be reviewed by OHA on an ongoing basis, and contracts may be issued at any time throughout the open application period.

Additionally, this document references a separate state and local contract opportunity for Trauma Hospital Surveyors, sought by the Oregon Department of Human Services, Public Health Division, Emergency Medical Services (EMS) and Trauma Systems. This opportunity is open to Emergency Physicians, Trauma Surgeons, General Surgeons, and Nurses with expertise in Trauma Program Management to conduct site reviews and focused reviews of Oregon hospitals to assess compliance with Oregon's trauma system statutes and rules. The contract opportunity is statewide, covering the jurisdiction of Benton County.
