About this file
The document is a Revised Cost Reply spreadsheet for the State of Florida's Department of Management Services (DMS) Invitation to Negotiate (ITN) No. DMS-24/25-232 for Group Term Life Insurance. The solicitation seeks a vendor to provide comprehensive life insurance coverage for approximately 345,037 members, including active full-time and OPS employees, vested legislators, retired employees, and their dependents. The contract will run from January 1, 2026, with an initial four-year term and potential for up to four additional one-year renewals. The cost reply document requires respondents to provide detailed premium rates for six different coverage types: basic group term life for eligible employees, vested legislators, retired employees, spouse life insurance, dependent child life insurance, and optional group term life insurance. Rates must be specified for initial terms (years 1-4) and renewal terms (years 5-8), with a weighted average calculation of 60% for years 1-4 and 40% for years 5-8.
The pricing structure includes various benefit options, such as $25,000 basic life insurance for active employees, up to $150,000 for vested legislators, and optional coverage ranging from 1-7 times annual earnings (maximum $1 million). Estimated annual enrollments vary by category, with the largest group being active employees. The cost reply spreadsheet allows for rates to be entered per $1,000 per month, broken down by age groups for optional life insurance. The document specifies that this revised cost reply will not be evaluated or scored during the initial phase but will be used by the negotiation team to establish a baseline for service and pricing discussions. The solicitation requires a performance bond of $5,000,000 and mandates integration with the state's existing People First system for data transmission and enrollment management.

D1 General Instructions
Instructions
Provide a rate in each yellow shaded cell in Tabs 2 through 7 for the initial and renewal terms. The Respondent's rates shall be to the third decimal place. The initial term rates are for years one (1) through four (4) and shall be the same rate for each initial term year. The renewal rates are for years five (5) through eight (8), and may be a different rate for each renewal term year. Tab D8 is an optional section where the Respondent can offer supplementary services and their corresponding rates provided by your company.
Tab D9 provides a summary of the Respondent's response along with weighted rates. The weighing of the initial and renewal terms and the benefits is described below.
D9 Summary Weighting
Premiums will be averaged over two time periods, Years 1 through 4 and Years 5 through 8. A weighted average will be calculated using a 60% weight for the Years 1–4 average and a 40% weight for the Years 5–8 average. The formula for this calculation is: (Average of Years 1–4 × 0.60) + (Average of Years 5–8 × 0.40).
Revised Cost Reply
As stated in Section 3.5 (Contents of Reply) of the ITN, this Revised Cost Reply will not be evaluated or scored during the evaluation phase of the ITN. The Department intends for these items to be utilized by the negotiation team to establish a baseline for service and pricing discussions with Respondent(s) during the negotiation phase of the ITN. Tab D9, in particular, will be utilized by the negotiation team to facilitate discussion and negotiation of pricing with respect to both initial and renewal terms of the contract (as opposed to discussing and negotiating pricing for each type of coverage separately for each of the initial and renewal terms).
