About this file
This is a Required Certifications form (PMT-58) issued by the Florida Department of Children and Families for an Invitation to Bid solicitation regarding Elevator Maintenance and Repair services in the Northwest Region. The solicitation seeks a qualified vendor to provide comprehensive maintenance and repair services for three hydraulic elevators located in Tallahassee, Quincy, and Marianna, including scheduled preventative maintenance, 24/7 callback services, emergency repairs, component replacements, and annual safety inspections in accordance with ASME A17.1 standards. Respondents must submit three mandatory documents: Required Certifications (PMT-58), Technical Response (PMT-59), and Cost Proposal (PMT-60). The pre-bid site visit period runs from November 24 through December 12, 2025, with responses due by January 8, 2026, at 2:00 PM and a public bid opening scheduled for 3:00 PM on the same date via video conference. The contract term extends for five years from February 1, 2026, through January 31, 2031, with the award going to the responsible and responsive respondent submitting the lowest responsive bid.
Respondents must hold certification from the Florida Building Code and Inspectors Board and be registered with the Florida Department of State. The contract includes monthly compensation with potential annual price adjustments capped at three percent, calculated using a formula allocating 80% to labor and 20% to materials. Contractors must provide comprehensive general liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate, along with workers' compensation and employer's liability insurance. The certification form requires respondents to affirm compliance with multiple Florida Statutes provisions, including conflict of interest, collusion, gratuities prohibitions, and vendor list exclusions. Respondents may claim tiebreaker preferences if they qualify as Certified Minority Business Enterprises, Certified Veteran Business Enterprises, Florida Businesses, Foreign Manufacturers with Florida factories, or Drug-Free Workplaces. The solicitation contains no specific set-aside designations or restrictions for disadvantaged enterprises, and the Department reserves the right to reject any or all responses.
