About this file

This is a Required Certifications form (PMT-58) for the Florida Department of Children and Families (DCF) Elevator Maintenance and Repair solicitation in the Northwest Region. Respondents must submit this certification document along with a Technical Response (PMT-59) and Cost Proposal (PMT-60) to be considered for the contract. The solicitation seeks a qualified vendor to provide comprehensive elevator maintenance and repair services for three hydraulic elevators at facilities in Tallahassee, Quincy, and Marianna. Services include scheduled preventative maintenance, 24/7 emergency callback services, component replacements, and annual safety inspections compliant with ASME A17.1 standards. The contract term is five years, from February 1, 2026, through January 31, 2031. Responses are due by January 8, 2026, at 2:00 PM, with a public bid opening scheduled for 3:00 PM the same day via video conference. An optional pre-bid site visit period is available from November 24 through December 12, 2025. The Department intends to award the contract to the responsible and responsive respondent submitting the lowest responsive bid.

The certification form requires respondents to confirm compliance with multiple statutory requirements under Florida law, including conflict of interest restrictions, non-collusion certifications, prohibitions on gratuities, and verification that the organization is not listed on excluded vendor lists maintained by the State of Florida and the federal System for Award Management (SAM). Respondents must also certify that their organization is not engaged in a boycott of Israel or business operations in Cuba or Syria. The form includes optional tiebreaking certifications for Certified Minority Business Enterprises, Certified Veteran Business Enterprises, Florida Businesses, Foreign Manufacturers with a Factory in Florida, and Drug-Free Workplaces. Respondents must hold certification from the Florida Building Code and Inspectors Board and provide general liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate. Monthly compensation is provided with potential annual price adjustments capped at three percent, with an allocation formula of 80% labor and 20% materials. Respondents must be registered with the Florida Department of State and complete a Florida Substitute Form W-9 prior to contract execution.
