About this file

This Quality Assurance Surveillance Plan (QASP) details the government's monitoring approach for a Municipal Solid Waste Disposal and Recycling Services contract for VISN 5 (Veterans Integrated Service Network 5). The document outlines specific performance standards across six key areas: container replacement, spill cleanup, monthly trash/waste poundage reporting, monthly recycling poundage reporting, invoice accuracy, and daily waste container servicing. Performance will be evaluated using periodic inspections and validated customer complaints, with ratings ranging from excellent to unsatisfactory based on response times and compliance levels.

The QASP establishes that the Contracting Officer (CO) and Contracting Officer's Representative (COR) will conduct semi-annual performance assessment meetings and document both acceptable and unacceptable contractor performance. Incentives will be based on the contractor's ability to meet or exceed performance standards, potentially impacting future option periods and past performance evaluations. If performance is deemed unsatisfactory, the government may issue a Contract Discrepancy Report (CDR) requiring the contractor to submit a corrective action plan. The document is designed to be a "living document" that can be reviewed and revised in coordination with the contractor.

• How monitoring will take place.

• Who will conduct the monitoring.

• How monitoring efforts and results will be documented.

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.

This QASP is a “living document” and the Government may review and revise it on a regular basis.

The CO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

The COR https://department.va.gov/integrated-service-networks/visn-05/ https://department.va.gov/integrated-service-networks/visn-05/ shall keep a quality assurance file.
