About this file

This is a Quality Assurance Surveillance Plan (QASP) for preventative maintenance of main switchgear equipment at the Northport VA Medical Center. The QASP outlines how the government will monitor and evaluate contractor performance through direct observation, cost proposals, and service reports.

The contractor must perform annual and recurring electrical maintenance with 98% acceptable quality level, provide repair proposals within 48 hours of emergencies, arrive on-site within 2 hours for emergencies (98% AQL), and respond to non-emergency repairs within 24 hours (95% AQL). All contractor staff must hold current Suffolk County Master Electrician Licenses (98% AQL). The COR will conduct surveillance through maintenance reports, response time tracking, and direct observation. Performance will be rated as either meeting or not meeting standards, with unacceptable performance documented through Contract Discrepancy Reports requiring corrective action plans. Performance assessment meetings will occur annually upon completion of preventative maintenance services, though the COR will meet monthly with the contractor to discuss ongoing performance.
