About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance for solicitation #36C24225Q0110, which relates to maintenance, testing, and repairs on high voltage equipment at Northport VA Medical Center for the Department of Veterans Affairs.

The questionnaire is divided into two parts: background information to be completed by the offeror and performance evaluation to be completed by the entity receiving services. The evaluation uses a five-tier rating scale (Outstanding, Good, Acceptable, Marginal, Unacceptable) to assess performance across multiple categories including quality of service, recruitment/retention, timeliness/schedule, business relationships, and customer satisfaction. Key evaluation areas include the contractor's compliance with regulations, ability to handle multiple sites simultaneously, contingency support capabilities, and effectiveness in managing critical requirements. The form also includes questions about previous contract terminations, cure notices, and willingness to award future contracts to the contractor.
