About this file
This is a Foreign Country of Concern Attestation form (PUR 1355) required by the Florida Department of Transportation for the Data Network Structured Cabling System project serving FDOT District 4 and District 6. The attestation must be completed and signed by an officer or representative of any entity submitting a bid, proposal, or reply for this contract, which would grant the entity access to personal identifying information. The form requires the signatory to declare under penalties of perjury that the entity is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not have a Controlling Interest in the entity. The attestation must include the printed name, title, signature, and date of the authorized representative and is governed by Rule 60A-1.020, Florida Administrative Code.
All bidders submitting proposals for the FDOT structured cabling project must complete and submit this attestation as a mandatory compliance requirement. Failure to provide this certification or providing false information may result in bid rejection and potential legal consequences under perjury statutes. The form serves as a critical compliance checkpoint for ensuring that vendor entities meet Florida's foreign ownership and control restrictions for contracts involving access to sensitive government information.
