Foreign Country of Concern Attestation (PUR 1355)
PUR 1355 is a Foreign Country of Concern Attestation form required by the State of Florida for the Florida Department of Transportation's data network structured cabling system procurement for District 4 and District 6. This attestation must be completed by an officer or representative of any entity submitting a bid, proposal, or reply for contracts that would grant access to an individual's Personal Identifying Information under Rule 60A-1.020, F.A.C. The form requires the signing officer to declare under penalties of perjury that the entity is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not have a Controlling Interest in the entity. The attestation requires the printed name, title, signature, and date of the authorized representative.
The form serves as a compliance document for the structured cabling project with a maximum budget of $65,000.00. All bidders must submit this attestation as part of their bid package, with electronic submission due February 25, 2026, at 10:30 AM. The attestation affirms the bidder's eligibility to handle sensitive information and compliance with Florida's foreign ownership restrictions, ensuring that only entities meeting these criteria can be awarded the contract.
