About this file
The PUR 1355 Foreign Country of Concern Attestation is a compliance form required by the Florida Department of Transportation for entities bidding on or entering into contracts that would grant access to personal identifying information. This attestation form must be completed and signed by an officer or representative of the bidding entity in connection with the District-Wide Mowing, Landscape Maintenance, and Debris Removal Services solicitation. The form requires the signatory to declare under penalties of perjury that the entity is not owned by, organized under the laws of, or has its principal place of business in a Foreign Country of Concern, and that no Foreign Country of Concern government maintains a controlling interest in the entity.
All bidders must complete and submit this form as part of their bid package, along with all other required attachments specified in the solicitation advertisement. Additionally, all prospective bidders must be registered in Florida's MyFloridaMarketplace system, and out-of-state entities providing services must be authorized to do business in Florida and possess any licenses required by the Florida Department of Business and Professional Regulation. The form serves as a critical compliance and security requirement to ensure that contractors handling sensitive personal information are not subject to control or ownership by foreign governments designated as countries of concern.
