About this file
This document is a Proposer Signature Page for a Request for Proposal (RFP) issued by the City of Kingman, Arizona. The signature page requires potential vendors to carefully review specifications and general provisions for a procurement opportunity, with the proposer agreeing to contract with the city to furnish specified items and/or services according to prescribed requirements. Proposers must complete and return several documents including a cover letter, contract terms and conditions, signature page, guarantees and warranties, and references. The proposal remains irrevocable for 120 calendar days from the RFP opening date.
The document outlines several key certification requirements for proposers, including non-discrimination against disadvantaged, minority, or women-owned small business enterprises, and a commitment to competitive submission without collusion. Proposers must certify they have not engaged in any agreements or actions that would restrain free competitive submission, and they may be required to provide a notarized "NO COLLUSION AFFIDAVIT" if requested. The signature page includes fields for the proposer to provide their name/title, signature, Arizona and Federal Tax ID numbers, company name, address, phone, and fax information, enabling the City of Kingman to formally document and track the proposal submission.
