About this file
This document is a Proposer Signature Page for a Request for Proposal (RFP) issued by the City of Kingman, Arizona. The form requires potential proposers to carefully review specifications and general provisions, with the intent to contract for specified items and/or services. Proposers are required to complete and return several documents, including a cover letter, contract terms and conditions, signature page, guarantees and warranties, and references. The proposal remains irrevocable for 120 calendar days from the RFP opening date, and proposers may withdraw their submission prior to the opening by providing written notice to the City Clerk.
The document outlines key proposer certifications, including a commitment to non-discrimination against disadvantaged, minority, or women-owned small business enterprises, and an agreement not to engage in collusive practices as defined by Arizona Revised Statute § 34-253. Proposers must be prepared to provide a notarized "NO COLLUSION AFFIDAVIT" if requested by the City. The signature page requires detailed company information, including name, title, signature, Arizona and Federal Tax IDs, company name, address, phone, and fax number, which must be completed by the proposing entity.
