About this file
This document is a Proposer Guarantees and Warranties form for a municipal contract, specifically involving the City of Kingman in Arizona. The form requires a proposer to certify their ability to provide services, comply with state laws, and provide truthful information about their business history and capabilities.
The questionnaire requires the proposer to disclose critical information about past performance, including whether they have incurred costs from contested change orders, been involved in litigation, failed to perform satisfactorily on previous contracts, or been convicted of criminal offenses that might impact their integrity as a municipal contractor. The proposer must also confirm they have adequate equipment, personnel, and expertise to complete the proposed contract. If the proposer answers "yes" to any of the first four questions or "no" to the fifth question, they are required to attach a full explanation to the document.
