About this file
This document is a Pricing Packet related to a Request for Proposal (RFP) issued by the State of Illinois for Conditional Release services. The RFP seeks to provide a continuum of community-based services, including assessment, treatment, and surveillance services for individuals released from secure facilities and placed within Illinois communities. The pricing packet outlines the format for submitting pricing, which includes monthly charges based on the number of releasees on the program, as well as average monthly expenses per resident. Vendors are required to submit a Phase-In Staffing Plan that details staffing positions, FTEs, salaries, and hourly rates. Vendors must also submit a detailed budget covering all relevant cost categories.
The contract term is for an initial period of 5 years, with a renewal option for an additional 4 years. The RFP does not mention any set-asides, incumbents, or specific funding requirements and sources.

· Staffing Position,
· FTE,
· Annual Salary,
