Matt Cockfield, Subcontract Administrator

This addendum, which is to be considered as if originally included in this RFP, makes the following changes:

Answers to questions submitted by potential offerors:

Q1. Per Section 2.1 of Exhibit D it states that the subcontractor shall have the capacity to process up to

1200 sample per week. Can you elaborate on the type of samples and number that are realistically expected on a weekly basis. Will these be mostly asbestos samples, PCB samples or something else?

A1. Here are some examples only. It varies quite a bit from month to month, as shown in the first chart.

Also, we have planned some soil sampling campaigns, as shown in the second chart mailto:Matt.Cockfield@em-la.doe.gov

There will be very minor IH samples per month: probably a maximum of 5.

Q2: Per Section 2.2.1 of Exhibit D it states that the subcontractor shall provide a U.S. EPA defined Level

IV report that included raw data. Can you verify that you are requesting this for all sample types including Industrial Hygiene samples? Typically IH samples are not required to have a Level IV data package. Additionally can you clarify if all samples will need Level IV reports or only specific samples by request?

A2: Industrial hygiene samples do not require level IV data packages.

Q3: Per Section 2.3 of Exhibit D it states that the subcontractor needs a current Radioactive Materials

License for handling at least 1.5 Ci. Can you confirm that this is the typically level that is expected to be seen with the samples coming in, or is this the worst case scenario? We do have different handling procedures based on the radiation level. Also, do you anticipate a difference in radiation levels between

Industrial Hygiene samples and Environmental samples?

A3: The 1.5 Ci is at the high end of our samples’ range. The environmental samples last year went up to

978 pCi/L (stormwater sample analyzed for gross alpha) and about 50 pCi/g (soil sample analyzed for Cs-

137). We anticipate that most of the IH samples we send will probably not be contaminated with radioactive material.

Q4: Per Section 3.1.1.3 of Exhibit D it states that all methods should be the most current promulgated versions. We run several NIOSH methods as modifications and are typically listed as such in our AIHA certification. Is this acceptable?

A4: Yes.

Q5: Per Section 3.3.1.9 of Exhibit D it states that MDLS for general inorganic chemistry and organic chemistry. Can you clarify if this applies to environmental samples only or does it include industrial hygiene samples as well. MDL studies are not typical for industrial hygiene samples as most method report to the PQL.

A6: The MDLs do apply to environmental samples; and reporting IH sample data with the PQL is acceptable.

Q6: Per Section 3.10.3.1 it states that temperature and custody seals shall be noted on the COC. Can you clarify if this applies to both environmental and industrial hygiene samples. For industrial hygiene samples we typically do not record the temperature for these samples.

A6: The temperature reporting and custody seals reporting do apply to environmental samples. In addition, there may be certain IH samples that will need temperature reporting or custody seals. For example, isocyanate IH samples will need temperature criteria met, and worker exposure samples

(breathing zone IH samples) will be shipped with custody seals.

Q7: Per Section 3.12.2.1 it states that there shall be 100% verification of agreement between EDDs and hard-copy reports. Can you verify that you are requiring hard-copy reports for all samples? Will a PDF suffice as a hard-copy, or does it need to be a print version? Or is this specific for environmental samples only?

A7: This is our mistake in the wording. We want analytical data package searchable PDFs rather than hard-copy reports.

Q8: Per Section 4.1.2.2 it states that instruments used to acquire general inorganic data shall be calibrated daily or once every 24 hours. Can you confirm if this is a full calibration or just a calibration check using a CCV?

A8: This is our mistake in the wording. Calibration of the instrument should follow the calibration requirements of the specific method.

Q9: Sections 5.2.10 and 5.2.11 address the use of data qualifiers. Can you confirm if this applies to only environmental samples? Industrial hygiene samples are not typically flagged.

A9: The data qualifiers apply to environmental samples.

Q10: Per Section 2.8 of the Mater IDIQ Agreement it states that subcontractor will submit weekly time reports. Does this apply in this case where samples are sent for analysis?

QA: This would only apply for services provided under Exhibit C, Section 9.1: Consulting and Other

Services.

Q11: The SOW in Section 1 includes radiobioassay samples in the sample descriptions and, in Section 3.2

DOELAP accreditation is noted for personnel monitoring programs; however, there are no radiobioassay analyses included in Exhibit C (Schedule of Quantities and Pricing). Will radiobioassay analyses be part of this contract and maybe added later?

A11: Radiobioassays analyses are not currently part of this contract, but may be added later.

Q12: In Exhibit C Under Special Preparations Section 7.0, please provide more detail for pay item 7.3

"Unknown sample characterization prior to standard analyses" so that pricing can be adequately established.

A12: An example of this is PCB or dioxin screening prior to congener analyses by a more sensitive method.

Q13: Reference Exhibit D Table 2-3: Radium-226 is listed as a gas flow test. Will this be reported as

Total Alpha Radium by EPA 903.0 Mod/SW846 9315 Mod?

A13: Yes.

Q14: For this RFP - is state of Utah accreditation specifically required for all analytical parameters as outlined in section 3.2 or would AIHA accreditation for asbestos supersede this and be sufficient?

A14: In the case of asbestos, the AIHA accreditation is sufficient.

Q15: Our firm is looking to respond for the asbestos portion only for the program. Would this be allowable?

A15: Yes. As per Exhibit C:

“ Please fill in the fields below as and where indicated (i.e. proposed analytical method, unit price, and multiplier) to the fullest extent possible. Note: it is anticipated that your lab will not be able to make an offer on all products/services; therefore, partial offerings will be considered for award.”

Q16: Is there any information available on quantities in regards to the specific required analyses?

A16: Please see tables for the answer for Question 2 for monthly samples.

Note: Answers to other bidder submitted questions submitted are forthcoming.
