About this file

This document is a Past Performance Questionnaire that collects information on a contractor's previous work performance. The questionnaire covers factors such as quality of work, personnel, cost/budget control, customer satisfaction, timeliness, and overall business relations. The contractor is asked to provide details on a specific contract, including the contract number, period of performance, award amount, and description of the work. The government evaluator then provides ratings ranging from Unsatisfactory to Exceptional for each performance factor, and indicates whether they would hire the contractor again. The document includes detailed rating guidelines to assist the evaluator in providing consistent and meaningful assessments.
