About this file
The file is a Quality Assurance Surveillance Plan (QASP) for a courier service contract with the Department of Veterans Affairs Veterans Health Administration (VISN 1). The QASP outlines comprehensive performance objectives and standards for evaluating the courier service provider, with six key performance metrics: timeliness of routine and STAT deliveries, accuracy of deliveries, condition of items, customer service, and compliance with safety and security protocols. Each metric has specific Acceptable Quality Levels (AQLs) ranging from 98-100%, with penalties for non-compliance including invoice reductions from 1-10% per percentage point below the established standard.
The surveillance methods include direct observation, inspections, documentation review, customer feedback collection, and formal audits. The Contracting Officer's Representative (COR) will assess performance monthly, documenting findings in performance reports. The plan provides a structured approach for monitoring service quality, with opportunities for the contractor to correct deficiencies and potential remedies including corrective actions, payment reductions, and potential contract termination for persistent underperformance. The QASP is designed to ensure high-quality, consistent courier services with transparent performance evaluation and continuous improvement mechanisms.

· Calculation of Invoice Reductions: Invoice reductions for non-compliance with performance metrics shall be calculated and applied to the monthly invoice.
· Opportunity to Correct: The Contractor shall have an opportunity to correct any identified deficiencies and demonstrate improved performance. Continued non-compliance may result in additional penalties, including and up to contract termination.
6. Communication and Reporting
· Communication Plan: The COR/CO will maintain open communication with the contractor, providing timely feedback and addressing concerns promptly.
· Reporting: Monthly performance reports will be generated and shared with both the contractor and the contracting office to ensure transparency and accountability.

8. Responsibilities
· Contracting Officer (CO): Overall responsibility for contract management and enforcement of the QASP.
· Contracting Officer’s Representative (COR): Day-to-day oversight of contractor performance and implementation of the QASP.
· Contractor: Responsible for meeting or exceeding the performance standards and requirements of the contract.
This QASP serves as a comprehensive guide to ensure that the courier service provider delivers high-quality service in a consistent and reliable manner. Regular monitoring, assessment, and feedback will help maintain the desired level of performance and foster a productive partnership between the contractor and the organization.
