About this file
This is a Past Performance Questionnaire for wastewater line repair services at Travis Air Force Base. The 60th Contracting Squadron is requesting that interested contractors complete the questionnaire and submit it by February 20, 2026, at 12:00 PM Pacific Standard Time to Contract Specialist Brandon Spratt (brandon.spratt.2@us.af.mil). The questionnaire will become part of the official source selection records for solicitation FA442726R0005, a Firm-Fixed Price, Total Small Business Set-Aside valued between $250,000 and $500,000 for repairing the sanitary sewer system under Building 650.
The questionnaire requests detailed information about the contractor's past and present performance on relevant contracts, including contract name/number, performance period, annual and total contract values, and the contractor's role (prime or subcontractor). Respondents must rate contractor performance across ten evaluation criteria using a five-point scale (1=Substantial Confidence to 5=No Confidence), addressing factors such as quality of service, responsiveness to problems, ability to recruit and retain qualified personnel, timely corrective action, management effectiveness, government cooperation, personnel credentialing, and overall contractor performance. Any ratings of 4 or 5 require narrative explanations. Additionally, the questionnaire asks whether the government would award another contract to the contractor. Confirmation of receipt should be directed to SrA Akeim Griffiths (akeim.griffiths@us.af.mil) or Brandon Spratt.
