RE: Hazardous Materials Survey 728 Hathaway Road Building, Vancouver, Washington

At your request, Kelsay Environmental performed hazardous material survey at the property located at 728 Hathaway Road in Vancouver, Washington. The survey was conducted on April 14th, 2015 and was performed to facilitate renovation of the old barracks structure. The structure onsite consisted of a large wood framed structure with a central hall and rooms on either side of the hall. The building also had a safe room that has a suspect asbestos door on the outside and the room was framed in concrete as was the boiler room that was below the main level of the building. The crawlspace at the site had suspect piping from the boiler room system.

Seventeen suspect materials were collected from throughout the structure they include: gypsum wallboard/joint compound (2 types), lay-in ceiling tiles vinyl tile/mastic (3 types), window glazing, covebase/mastic (2 types), CMU mortar, brick mortar, magnesia pipe insulation and composition roofing shingles.

Only the magnesia pipe insulation tested positive for asbestos. Prior to demolition or renovation, asbestos-containing building materials are required to be removed by a certified abatement contractor per Labor and Industries and WISHA regulations. No other suspect asbestos-containing building materials were observed. The following Table summarizes positive materials their quantities and estimated abatement cost.

Kelsay Environmental conducted a lead paint investigation, a representative screening for the presence of lead paint consisting of 2 paint chip samples. Paint sampling was performed to assess the hazard and not all surfaces were sampled. The exterior sample tested positive for paint at around 31,471 ppm lead content. Sample results are listed below in Table 3.

Please find the attached tables and lab results. Should you have questions or need anything further please call me at 503-705-0514.

Sincerely, Brad Kelsay Project Manager

LEAD INSPECTION SUMMARY

Two samples were collected for lead as a lead paint screening. Paint sampling was performed to assess the hazard and not all surfaces were sampled. Based upon the results all painted surfaces at the site should be presumed to contain some lead. A summary of the sample results are listed below in Table 3. All demolition/selective demolition must be performed in accordance with OSHA 1926.62 and WAC 296-155-176 requirements for the safe handling of lead-containing materials.

TABLE 3

PCB BALLAST/MERCURY INSPECTION

Poly Chlorinated Biphenyls (PCB) are a dielectric oil used in transformers, light ballasts and other electrical devices. PCB’s caused cancer and persist in the environment and must be disposed of under EPA guidelines.

A screening for PCB-containing ballasts was conducted. No PCB ballasts were found after inspecting a representative sampling. All ballasts to be removed should be verified to be Non-PCB before disposal. If PCBs are found to contain PCBs the must be disposed of per EPA- TSCA 40 CFR Ch.1 part 761.

All fluorescent light tubes (Approximately 170 light tubes) contain mercury and should be recycled and/or disposed observing regulations. Care should be taken to prevent breakage of the tubes during removal as vapors/gases will be released.

Materials Characterization - Bulk Asbestos Analysis Laboratory Analysis Report - Polarized Light

Kelsay Environmental Brad Kelsay 3693 SE Francis St.

Portland, OR

Fort Vancouver 710 Building

CBR15041098

Summary of polarizing light microscopy (PLM / Stereomicroscopy bulk asbestos analysis) using the methods described in 40CFR Part 763 Appendix E to Subpart E (Interim and EPA 600 / R-93 / 116 (Improved). The sample is first viewed with the aid of stereomicroscopy. Numerous liquid slide preparations are created for analysis under the polarized microscope where identifications and quantifications are preformed. Calibrated liquid refractive oils are used as liquid mouting medium. These oils are used for identification (dispersion staining). A calibrated visual estimation is reported, should any asbestiform mineral be present. Other techniques such as acid washing are used in conjugation with refractive oils for detection of smaller quantities of asbestos. All asbestos percentages are based on calibrated visual estimation traceable to NIST standards for regulated of asbestos.

Traceability to measurement and calibration is achieved by using known amounts and types of asbestos from standards where analyst and laboratory accuracy are measured. As little as 0.001% asbestos can be detected in favorable samples, while detection in unfavorable samples may approach the detection limit of 0.50% (well above the laboratory definition of trace).

Discussion

Qualifications

Vermiculite containing samples may have trace amounts of actinolite-tremolite, where not found be PLM should be analyzed using TEM methods and / or water separation techniques. Suspected actinolite/vermiculite presence will be indicated through the sample comment section of this report.

Fibrous talc containing samples may even contain a related asbestos fiber known as anthophyllite. Under certain conditions the same fiber may actually contain both talc and anthophyllite (a phenomenon called intergrowth). Again, TEM detection methods are recommended. CA Labs PLM report comments will denote suspected amounts of asbestiform anthophyllite with talc, where further analysis is recommended.

Some samples (floor tiles, surfacings, etc.) may contain fibers too small to be delectable by PLM analysis and should be analyzed by TEM bulk protocols.

A "trace asbestos" will be reported if the analyst observes far less than 1% asbestos. CA Labs defines "trace asbestos" as a few fibers detected by the analyst in several preparations and will indicate as such under these circumstances.

Quantification of <1% will actually be reported as <=1% (allowable variance close to 1% is high). Such results are ideal for point counting, and the technique is mandatory for friable samples (NESHAP, Nov. 1990 and clarification letter 8 May 1991) under 1% percent asbestos and the "trace asbestos". In order to make all initial PLM reports issued from CA Labs NESHAP compliant, all <1% asbestos results (except floor tiles) will be point counted at no additional charge.

CA Labs is accredited by the National Voluntary Accreditation Program (NVLAP) for selected test methods for airborne fiber analysis (TEM), and for bulk asbestos fiber analysis (PLM). All analysts have a college degree in a natural science (geology, biology, or environmental science) or are recognized by a state professional board in one these disciplines .Extensive in-house training programs are used to augment education background of the analyst. The group leader of polarized light has received supplemental McCrone Research training for asbestos identification. This report is not covered by the scope of AIHA accreditation.
