EXECUTIVE SUMMARY
Environmental Planning Management Inc EPM as a subconsultant to HDR Engineering
Inc performed a hazardous materials survey for asbestoscontaining materials leadbased paint radon and universal wastes of all accessible areas within the Theodore Roosevelt
Birthplace National Historical Site THRB located at 28 East 20`h Street New York New York This survey was requested in response to proposed HVAC system upgrades and future fire protection electrical system and ADA compliance renovation projects under consideration by the National Park Service NPS The leadbased paint survey is provided as Volume 11
Theodore Roosevelt lived in the original brownstone on this site from his birth on October 27 1858 until he was 14 years old After Roosevelts death in 1919 prominent citizens purchased the site demolished the commercial building that had replaced the original building and reconstructed Roosevelts boyhood home as a memorial
The approximate 18 acre site is occupied by a building comprised of east and west properties that appear on the exterior to be two adjoining townhouses The building is four stories high with a raised basement The interiors of the two buildings are jointed on each floor with doors through the party walls The rear exterior of the building has a steel structure fire escape staircase with no exit from the rear area There is a second floor terrace abutting Exhibit Hall
11EH11 and a patio roof over the fourth floor Auditorium No record plans addressing the presence of asbestoscontaining materials leadbased paint or universal wastes were located in the documents available One reference to installation of asbestos roofing in 1954 was identified however it is not known which specific roof area was addressed
The field inspections were conducted on November 13 through the November 29 2006 by
Environmental Planning Management certified New York State Department of Labor
NYSDOL asbestos inspectors Michael Aprahamian All 9702855 Andrzej Zabrocki AH 9320787 and Joseph Rizzo AH 0614089 The leadbased paint survey Volume II was conducted by Certified EPA Lead Risk Assessor John Rutledge NYR122561
The purpose of the survey was to identify the location and quantity of hazardous materials in preparation for renovation and rehabilitation activities with the potential to impact these materials
Predesign Report
An August 2006 predesign report prepared by Leo A Daly Co Daly identified the HVAC fire protection electrical system and ADA compliance deficiencies present at the THRB Although this report did not include a hazardous materials survey the report did identify the potential for the existence of hazardous materials ie peeling paint degrading pipe insulation According to the Daly report no comprehensive hazardous materials survey had been performed for THRB A limited asbestos investigation was performed by HA Bader
Consultants Inc Bader in March 2006 to identify ACM that may be impacted by the north
Environmental Planning Management i National Park Service facade restoration The ACM identified by Bader was abated prior to the facade restoration which was completed in January of 2007 EPMs asbestos survey encompassed all accessible interior and exterior areas of the building with the exception of the exterior front facade which was previously addressed

20 ASBESTOS SURVEY
This section describes data collection activities including the site inspection and sample collection for the asbestos survey
The field inspections were conducted on November 13 through the November 29 2006 by
Environmental Planning Management certified New York State Department of Labor
NYSDOL asbestos inspectors Michael Aprahamian AH 9702855 Andrzej Zabrocki AH 9320787 and Joseph Rizzo AH 0614089 Copies of EPM company license and personnel certificates are presented in Appendix A
21 Scope of Overall DesignPurpose of Asbestos Survey
The proposed upgrades to the HVAC system are in the predesign stage thus specific areas of impact have not been ruled out The August 2006 Leo A Daly report recommended that a
Hazmat inspection be performed A limited asbestos investigation was perform by HA Bader
Consultants Inc in March 2006 to identify ACM that may be impacted by the north facade restoration The ACM identified by Bader was abated prior to the facade restoration EPMs asbestos survey identified encompassed all accessible interior and exterior areas of the building with the exception of the exterior front facade The facade restoration was completed in
January of 2007
22 Record Plan Review Previous Environmental Investigation
No record plans pertaining to asbestoscontaining materials were located in review of available documents Historical records indicate that in 1954 a specific reference to roof work with tiling and waterproofing with asbestos fibre cement although the specific roof location is unknown With the exception of the limited investigation performed by Bader for the facade work no previous environmental investigations or surveys have been performed
23 Field Inspection
231 Sample Collection
A strategy for sampling suspect asbestos containing materials was developed employing the delineation and grouping of homogeneous suspect materials The delineation of homogeneous areas at the site was based on several criteria including material type and location Materials suspected of containing asbestos were identified for the area inspected When suspect ACMs were found representative bulk samples from the homogeneous material group material which is uniform by color texture construction application date and general appearance were collected Three bulk samples were collected per homogeneous material group from miscellaneous materials such as vinyl floor tiles and associated mastic The Asbestos Hazard
Emergency Response Act AHERA 357 rule was utilized for the sampling of surfacing materials ie wall and ceiling plasters and sprayedon fireproofing Samples were typically
Environmental Planning Management 2 National Park Service collected at a rate of three bulk samples per homogeneous area for thermal system insulation materials Based on National Park Service requirements one quality control QC sample was collected per 20 samples
232 Field Survey Observations
During the asbestos survey our team inspected all areas that could potentially be impacted by the proposed renovations EPM personnel coordinated our sampling effort with Mr Steve
Laise THRB Park Historian and Mr Mike Darden Site Manager Supervisor Park Ranger of
NPS Every effort was made to minimize disturbing historical architectural construction museum displays and or design elements
EPM personnel observed that at some point in the past removal of suspect pipe insulation PI and mudded joint packing MJP was performed in two mechanical crawl spaces in mechanical rooms 7 and 8 MR7 MR8 in the rear of the subbasement of the building This abatement was incomplete and improper based on the residual asbestos observed on the piping and on the floors Remnant residue collected by EPM and analyzed by the laboratory tested positive for asbestos In addition new fiberglass pipe insulation was placed over the improperly abated steam pipes in MR7 Samples of the debris piles on the floor in MR7 and MR8 were collected and also tested positive for asbestos Pipe insulation and MJPs are present throughout the building in crawl spaces in the 3rd and 4 floors vertical risers roof drains and steam pipes associated with the radiators All doors in the subbasement are metal covered wood core Doors throughout the building are solid wood or metal covered wood doors

Samples in a homogeneous group are analyzed until either the entire group is analyzed all the results are negative or a positive result is obtained by PLM When a positive result occurs the remaining samples in the group are not analyzed If the results are negative by PLM and the material is nonfriable organically bound the first sample in each homogeneous group is analyzed by TEM A total of 255 bulk samples were collected from seventyeight 78 homogeneous groups of suspect asbestoscontaining materials at THRB and submitted for laboratory analysis EPM collected a total of thirteen 13 QA samples for laboratory analysis
The electrical wiring at the THRB is a combination of older suspect ACM cloth woven braided wiring insulation and newer nonACM PVC or plastic insulation Since this wiring could not be deenergized it is assumed asbestos containing wire insulation until proper analysis of the braided wire insulation can determine otherwise Several areas where assumed ACM braided wire insulation was observed are as follows o Various wires located in the relay assembly and fuse panel located inside
MR4 elevator machine room o The west wall of the MR1 where the central vacuum system is located o Dumbwaiter Elevator Mechanical Room various wires associated with elevator motor relay panel and disconnect panel on the north wall o Wiring associated with light fixture located inside HVAC room on the 5th floor in the southwest area o Various wires inside large electrical panel located on the 4`h floor backstage in storage rooms ST5 o Various wires inside breaker panel located on 4`h floor loft level in the projection room

Elevator motors use a braking system similar to a car to stop the motors EPM personnel observed suspect brake pad material at the motor in MR4 and in the dumbwaiter elevator mechanical room above the south bulkhead roof Since the system could not be deenergized the brake pads are assumed asbestos containing until proper analysis of the brake pads can determine otherwise In addition assumed asbestos arcshield material was observed in the relay assembly in MR4 and the disconnect panel associated with the central vacuum system in
MR1 Black ebony backing material present in the relay assembly and fuse panel in MR4 is made of slate This material is not suspect for asbestos This slate material is also present in electrical panels associated with the vacuum system electrical panels in MR1 and the electrical panels in the dumbwaiter elevator mechanical room on the bulk head roof
Samples were collected of the black ebony backing material present in the electrical panels in ST5 and the projection room This material is not slate and tested positive for asbestos
Electrical panels in MR5 are of newer design and EPM did not observe suspect ACM in these panels One large panel and one smaller panel in MR5 are locked and is the property of Con Edison EPM did not have access to the interiors of these panels
EPM inspected and sampled materials which may be impacted by future renovations under consideration and all observed suspect building materials During the asbestos survey EPM did not perform destructive testing of any walls or ceilings There is the possibility that additional suspect asbestos containing materials are present within concealed or inaccessible spaces that have not been identified Once an HVAC design is established any suspect asbestos containing materials encountered during the course of the work which have not been tested must be assumed ACM until laboratory analysis indicates otherwise
233 Radon Testing
Radon testing is part of the hazardous materials survey Since the THRB is not located in a high risk radon area EPM conducted passive radon testing Two charcoal canisters were set up as follows
RI was placed at the northern end of mechanical room2 MR2 located in the subbasement
R2 was placed in the northwest corner of officel 01 located in the basement
The two canisters were placed at locations described above on November 13 2006 at 1100
AM and left for seven consecutive days for the exposure period
The following sampling protocol was followed The canisters were placed in the approximate center of their immediate locations R1 was hung from a drain pipe approximately six feet from the floor R2 was hung from angled plaster ceiling approximately 5 feet from the ground On
November 20 2006 at 1100 AM the canisters were retrieved and submitted for laboratory analysis
