The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate
(weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)
and 3.) should be followed.
With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:
Branch of Construction Wage Determinations Wage and Hour Division U.S. Department of Labor 200 Constitution Avenue, N.W.
Washington, DC 20210
2.) If the answer to the question in 1.) is yes, then an interested party (those affected by the action) can request review and reconsideration from the Wage and Hour Administrator (See 29 CFR Part 1.8 and 29 CFR Part 7). Write to:
Wage and Hour Administrator U.S. Department of Labor 200 Constitution Avenue, N.W.
Washington, DC 20210
The request should be accompanied by a full statement of the interested party's position and by any information (wage payment data, project description, area practice material, etc.) that the requestor considers relevant to the issue.
3.) If the decision of the Administrator is not favorable, an interested party may appeal directly to the Administrative Review Board (formerly the Wage Appeals Board). Write to:
Administrative Review Board U.S. Department of Labor 200 Constitution Avenue, N.W.

Subject: Amendment No. 001 Questions & Answers
Please find below responses to questions in red.
1. Per General Note #3 on sheet G1, work is to be coordinated with NPS to avoid damage to existing historic aircraft within hangar. Are any protective measures to be included in contractors cost for protection fo aircraft? Will the aircraft be relocated for contractor to safely perform roof demolition operations (ie: wood deck removal)? - The National Park Service will move the historic planes to one side of the hangar while work is being completed on the opposite portion of the structure.
NPS will then move the planes to the side where work is complete so that the remaining section of the building can be completed. The Contractor shall be responsible for coordinating the schedule of work with NPS staff and assuring the planes will not be disturbed. Using this method, there should be no reason the contractor would need to incur costs for temporary protection of the planes.
2. Per abatement spec, roofing abatement is not part of scope. Please confirm Owner will be having this performed separately, or by a potentially requested unit price. - Owner is not performing any abatement. As per specification section division 2 and construction drawings, ACM has been identified on the existing canopy roofs in the transite panels. Contractor is fully responsible for all hazardous material abatement on this project.
3. Section 2/A11 is called out as "TYP" on sheet A6. This section on A11 seems to indicate the required detail if Option A is taken. If Option A is not taken, this detailed wouldn’t apply. Please confirm, or clarify. - Reference wall section B/A11 for basic scope verses option A at west lean-to wall.
4. Drawing A4 notes replacement of light fixture lamps, however there is no specification for electrical work. Please confirm this is part of the contract scope. - As per construction drawing A4, contractor is responsible to re-lamp existing fixtures as identified in key note #1 and #2.
5. At areas where additional glass pane breakage is indicated, is the percentage to be applied to the amount already indicated to be replaced, or the entire existing glass area. - The additional glass pane breakage percentage should be applied to the amount called out to be replaced.
6. Please confirm painting of interior / exterior hangar door upper portions includes window mullions (both horizontal & vertical members). - Interior and exterior of doors including the window mullions is included.
7. Please clarify whether only unit prices are to be provided for concrete wall repairs (details 2 & 3 on S6), OR whether unit prices, as well as the lump sum amount for repair quantities listed at details are to be provided. Lump sum amount is assumed to be a part of the overall bid in second scenario. - For the purpose of bidding, the repair quantities called out in the details should be bid on as a lump sum amounts. Unit prices should be provided in case the actual quantities turn out to be either more or less than the assumed quantities once all conditions are exposed.
8. Please clarify whether east lean-to window sills are to be cast-in-place or precast. Sheet S1 calls these sills to be "C.I.P.", whereas sheet S7 calls to be precast.- East Lean-to window sills should be pre-cast.
9. Detail 1 on A10 indicates new metal roof flashing to be installed behind existing panels. For this to occur, removal of the panels will be required; please confirm. - If flashing cannot be slipped in behind and secured with the asbestos containing transite panel in place, then extreme care must be exercised to not disturb or damage the panel during removal and reinstallation.
10. Please clarify spacing of unistrut post at roof per detail 4/A10. Is this post assembly to be installed at each leader or are other spacing requirements to be followed? - As per keynote W9 on A5, the spacing should be 8’-0” O.C.
11. Page 9 of 132 of the attachment titled "P15PS00421.pdf" lists several items which are not called out in the contract documents. Unless clarified, it will be assumed these are part of other projects at this building. Items not clear in the documents include:
a). West Lean-to Interior - Clean area (two floors)…Replace water damaged walls and removed water damaged ceilings (6) Not in scope.
b). West Lean-to Roof - …repair steel… (7) Drawings indicate to scrape and paint top of steel flanges only True for roof steel under roof deck.
c). East Lean-to Roof - …repair steel… (8) Drawings indicate to scrape and paint top of steel flanges only True for roof steel under roof deck.
d). Miscellaneous Repairs - …repair or replace tracks [hangar doors](10,d) We did not call for replacement of tracks, only removal of rust, cleaning and greasing.
f). Miscellaneous Repairs - Repair interior hanging lights to be operational (10,f)Drawings indicate to relamp only -- no repairs to fixtures. There is no electrical spec. As per question #4, only relamping is called for.
12. Please clarify glass types to be installed at the following locations -- Drawings / specs do not clearly note which glass type goes where, just the different glass types:
a). Clerestory windows - Clear tempered glass to match existing thickness, ¼” minimum b). Replacement aluminum windows - 1” insulated glass units.
c). Restored steel windows- Clear tempered glass to match existing thickness, ¼” minimum d). Monitor windows - Clear tempered glass to match existing thickness, ¼” minimum
13. Please clarify scope of Key Note M1 on sheet A5. As part of this item, will the assumed 9,300SF be sealed, primed and painted, OR just the locations that are patched with corrugated aluminum to be primed/painted to match?- It should be assumed that 9,300 SF of transite paneling will need to be patched with aluminum paneling, sealed, primed and painted. Locations may be discovered that are not necessarily shown on the drawings.
14. Please clarify scope required for making hangar doors operational, per note on sheet A1. Please confirm this is limited to cleaning of rust, greasing tracks and installing weather stripping per drawings. - Same questions as 11d. Scope is limited to the contract documents.
15. Can the north tunnel water being removed be discharged on-site? If so, will it need to be run through a frac tank? - North tunnel contains rain water that may have traces of oil and must be disposed of off site.
16. Please provide approximate dimensions of north tunnel (specifically depth). - Tunnel is shown on the floorplans and runs the length of the building with an approximate depth of 6 feet.
17. Please clarify scope relating to vent piping thru roof at East lean-to. Sheet DM1 indicates to "cut off and cap exg vent below roof deck, typ at E-Lean-to". Sheet A3 indicates existing vent pipe. Is the intent to cut off the vent pipes and then install new vent pipes from below the roof to above the roof OR to leave the existing vent pipes in place? - At the East Lean-to the intent is to cut off existing vent pipes below roof level and cap, leaving them in place. No new vent pipes are in scope nor will the existing vent pipes penetrate the new roof.
18. Please clarify spacing for drilling holes at upturned channel of monitors to be drilled. Sheet S4 indicates
8' OC, whereas detail 1/S3 indicates 8" OC. - As per Sheet S4, the spacing of the drainage holes should be 8 feet on center.
19. Regarding Hangar Doors:
A. What does greasing the steel tracks involve? Grease on the bottom tracks will not help the doors move easier. It will also create a mess that will attract dirt and get on anyone pushing the doors.
B. Does removal of rust mean sand blasting? Is there lead based paint on the door tracks?- Scope of work to clean and grease hangar doors is noted on 3/A12. Tracks are not painted.
20. Please clarify the amount of on-site personnel the contractor is required to have. Are any of the below responsibilities able to be shared across contractor staff? And are they required full-time?
- Contractor's Environmental Manager (01 31 00, 1.8, A) Quality Control Supervisor (01 40 00, 1.5, B) -- Can this individual be the project supervisor, if qualified?
- Required on-site personnel responsibilities can be performed by the same individual if qualified.
21. The SF 1413 requires the proposal to be submitted on 3/23/15. The RFP documents requires the proposal to be submitted on 3/20/15. What is the response date? –Proposals are due no later than 4:00 PM MST on 3/23/15.
22. The hanger door bottom rails are severally deteriorated in several locations. It was hard to really see how bad they were with the ice that had built up around them. Per drawing sheet A1, it calls for rust removal and greasing of steel tracks. You should not grease these tracks because it would be safety issue and that is not a standard in the hanger door industry. There are 2 ways of replacing these tracks.
Option 1- demo the concrete to approx. 10” below finished floor, replace supports and rails and then re-pour new concrete. This is quite expensive and may not need to be done.
Option 2- saw cut and replace the bottom rail sections only. This can be done by removing the rail only and reinstalling new rails on the existing supports. We will then install grout around the new rail and form to the existing. Much cheaper then option 1.
Again, it is very hard to tell how many linear feet need to be replaced with the amount of ice that was built up in the rail. Would either one of these options be expectable? - The scope calls out to repair or replace the hangar door tracks. The intent is severely damaged track that cannot be cleaned will be replaced in kind.
23. We suggest replacing all the bottom wheels and bearings. Many of the bottom wheel flanges are rolling over and the doors are going to come off the rails in the future. Some of the doors have pickup brackets on them. Do you want all new pickup brackets installed? - Scope of work on hangar doors is limited to the contract documents.
24. Some of the contractors would like another site visit, is there someone at the hanger we may contact to set up a date and time? - The Pre-bid meeting was held on 3/2/15 and there are no additional site visits.
25. The roof drawings doesn’t show ½ recovery boards. This is required by manufactures for warranties, Should the entire roof be covered with ½ recovery board? Scope of work on the roof is limited to the contract documents.
26. Section 52.219-4 refers to Price Evaluation Preference for HUBZone Small Business Concerns. We are not located in a HUBZone. Does this mean that 10 per cent will be added to our bid price? Per Section 52.219-4, the price evaluation preference is for HUBZone Small Business Concerns. The definition was a HUBZone Small Business Concern is found in FAR Part II, “a HUBZone Small Business Concern is a small business concern that appears on the List of Qualified HUBZone Small Business Concerns maintained by the Small Business Administration”.

1.0 Introduction
As authorized by Mr. George Beckwith of Beyer, Blinder, Belle, Froehling & Robertson, Inc.
(F&R) personnel performed a limited, non-invasive asbestos survey at Hangar B located at Floyd Bennett Field in Brooklyn, New York on June 10, 2014. The scope of work was limited to the interior of the west side second floor and the roof as directed by the proposed emergency stabilization planned for the Hangar. The sampling was performed by Mr. Andre Steuer, Industrial Hygienist with F&R.
2.0 Asbestos-Containing Material (ACM)
2.1 Methodology
For this project, a visual, non-invasive survey and sampling for suspect asbestos containing materials was conducted at the above referenced site, as specified by the Client. All samples were collected by an EPA trained asbestos inspector and submitted to EMSL Laboratories a NVLAP Accredited Asbestos Laboratory, located in Beltsville, Maryland. The stop positive method of analysis was utilized, whereas if one sample from a homogenous group is determined to be asbestos-containing, additional samples from that homogenous group are not analyzed. A total of sixty-two (62) sample layers were collected and then analyzed using Polarized Light Microscopy (PLM) following EPA Method 600/R-93/116.

2.3 Conclusions and Recommendations
2.3.1 Friable Asbestos-Containing Materials
Friable asbestos-containing materials were not detected during this site visit.
2.3.2 Non-friable Asbestos-Containing Materials
Transite Siding and Railing: 20%-30% Chrysotile
Asbestos was detected in the samples collected of the transite siding and railing on the exterior of the hangar and the second floor railing, during F&R’s June 10, 2014 site visit. This material is classified as a Category II Non-friable Asbestos-Containing Material by the United States Environmental Protection Agency (USEPA) under the National Emission Standards for Hazardous Air Pollutants (NESHAP), 40 CFR 763. This material was observed to be in good condition at the time of the survey. All transite panels on the exterior and interior of the hangar should be assumed to contain asbestos. Approximately 4,800 square-feet of this material is expected to be removed as part of the emergency stabilization at the Hangar. The remaining transite is not expected to be impacted
Floor Tile and Mastic: 2%-6% Chrysotile
Asbestos was detected in the samples collected of the floor tile and mastic on the second floor of the west side of the hangar, during F&R’s June 10, 2014 site visit. This material is classified as a Category II Non-friable Asbestos-Containing Material by the United States Environmental Protection Agency (USEPA) under the National Emission Standards for Hazardous Air Pollutants (NESHAP), 40 CFR 763. This material was observed to be in good/fair condition at the time of the survey. All floor tile and associated mastic on the west side second floor should be assumed to contain asbestos. Approximately 18,000 square-feet of this material is expected to be removed as part of the emergency stabilization at the Hangar.
Fill Material in Corrugated Transite Siding 8% Chrysotile
Asbestos was detected in the sample collected of the fill material in some of the ridges on the transite panels on the roof and exterior of the structure, during F&R’s June 10, 2014 site visit.
Any filling located within the ridges of the transite panels should be assumed to contain asbestos. An unknown quantity of this is provided as not all ridges had this fill material.
2.3.3 Suspect/Presumed Asbestos-Containing Materials
All other suspect/presumed materials in the building are assumed to be asbestos containing material until sampling is conducted.
Note: If during renovation/demolition, work is performed that will impact any suspect/presumed materials, including those located behind solid walls and/or above solid ceilings and the exterior that have not been sampled, it is recommended that these materials be sampled by a Virginia Licensed Asbestos Inspector prior to disturbance.
2.4 EPA / NESHAP Regulations for Asbestos Containing Materials
The U.S. Environmental Protection Agency promulgated the National Emission Standards for Hazardous Air Pollutants (NESHAP) [40 CFR Part 61], which addresses the application, removal, and disposal of asbestos-containing materials (ACM). Under NESHAP the following categories are defined for asbestos-containing materials:
Friable - When dry, can be crumbled, pulverized, or reduced to powder by hand pressure.
Non-friable - When dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
Category I Non-friable ACM - Packings, gaskets, resilient floor coverings, and asphalt roofing products containing more than 1% asbestos.
Category II Non-friable ACM – Any material, excluding Category I Non-friable ACM, containing more than 1% asbestos.
Regulated Asbestos Containing Material (RACM) – One of the following:
1. Friable ACM
2. Category I Non-friable ACM that has become friable.
3. Category I Non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading.
4. Category II Non-friable ACM that has a high probability of becoming, or has become, friable by the forces expected to act on the material in the course of demolition or renovation operations.
Under NESHAP, the following actions are required:
1. Prior to the commencement of demolition or renovation activities, the building owner must inspect the affected facility or part of the facility where the demolition or renovation activities will occur for the presence of asbestos.
2. Remove all RACM from the facility before any activity begins that would break up, dislodge, or similarly disturb the material or preclude access for subsequent removal.
3. RACM need not be removed if:
a) It is Category I non-friable ACM that is not in poor condition.
b) It is on a facility component that is encased in concrete or other similar material and is adequately wet whenever exposed.
c) It was not accessible for testing and was therefore not discovered until after demolition began and because of the demolition the material cannot be safely removed.
d) It is Category II non-friable ACM and the probability is low that the material will become crumbled, pulverized, or reduced to powder during demolition.
4. If Category I and Category II Non-Friable ACM’s stay in-place during demolition, then all demolition debris should be disposed of at a landfill that accepts these wastes.
Furthermore, if these materials stay in-place during demolition, then some building materials may not be able to be recycled.
