About this file

This is an Invitation for Bid form submitted as an offer to the State of Illinois, specifically for the Illinois Emergency Management Agency - Occupational Health and Safety (IEMA-OHS) Diesel Trash Pumps procurement. The document serves as the standard bidding form through which vendors formally submit their offers to the State of Illinois for consideration. The form requires bidders to complete and submit all required documents as outlined in solicitation document section A.1, with the understanding that failure to meet all requirements will result in disqualification from consideration.

The form addresses bidder preferences available under the Illinois Procurement Code, including the Illinois Business Preference and the Domestic Product Preference, and instructs vendors claiming these preferences to complete appropriate affidavits and documentation accordingly. Bidders are also permitted to take exceptions to the Standard Terms and Conditions by submitting a redlined version, which the State may accept or reject at its discretion. The Standard Terms and Conditions that will govern any resulting contract are available through the Illinois Chief Procurement Office website. By completing and signing the form, the bidder makes a binding offer to the State of Illinois that constitutes acceptance of the solicitation terms unless exceptions are noted.
