About this file
This document is a Supporting Forms and Questionnaires Exhibit from the New York State Department of Transportation (NYSDOT) Purchasing office for use with Invitations for Bids (IFB) and Requests for Quotations (RFQ). The exhibit outlines the required forms and certifications that vendors must complete and submit with their proposals. Required submissions include the Procurement Lobby Act Offer Certification Form, Executive Order 177 and 16 Certification Forms, Vendor Assurance of No Conflict of Interest or Detrimental Effect Form (for service contracts only), Manufacturer's Certification Form (for commodity contracts), and the Affidavit of No Change Form if applicable. Additionally, vendors must complete the Vendor Responsibility Questionnaire (VRQ) obtainable through the Office of the State Comptroller website, and if the bid includes prevailing wage requirements, must be registered with the New York State Department of Labor prior to submission. NYS Sales and Use Tax forms (ST-220-CA and ST-220-TD) are required and available through the NYS Department of Taxation and Finance or upon request from the NYSDOT Purchasing Office.
The specific contract opportunity referenced is RQ26-004 for aggregates for maintenance use on vendor-placed paving projects in St. Lawrence County. This is a discretionary purchase under State Finance Law section 163(6) and may include set-asides for New York State small businesses, businesses certified as minority or women-owned enterprises under Article 15-A of the Executive Law, and service-disabled veteran-owned business enterprises (SDVOB). Interested vendors should contact Jennifer Walter at (518) 457-9434 or Jennifer.Walter@dot.ny.gov as the primary contact, or Relina Sosa at (518) 485-5192 or Relina.Sosa@dot.ny.gov as the secondary contact. All communications and bid submissions must be directed to the designated contact persons to avoid procurement lobbying violations.
