About this file

This document is Addendum #5 for the Nome Hangar Remodel project, though the file appears to primarily contain details about a separate State of Alaska procurement for a Government Performance and Results Act (GPRA) Portal. The Request for Proposals (RFP 2026-1600-0063) was released by the Municipality of Anchorage and Alaska Department of Health's Division of Behavioral Health on September 5, 2025, with questions due by October 10, 2025, and proposals due on October 20, 2025, at 5:00 PM Alaska prevailing time. The procurement seeks a Software-as-a-Service (SaaS) technology platform to collect client-level GPRA data for State Opioid Response (SOR) funding programs, requiring a HIPAA-compliant online data submission portal capable of capturing comprehensive client information across multiple domains.

The contract is structured for a five-year engagement and requires vendors to demonstrate at least three years of experience supporting GPRA data collection within the past five years. Bidders must maintain a current Alaska business license and have an established state business presence. While the Cost Proposal form shows deliverables priced at $0 (potentially a placeholder), the solution must meet federal mandates and Department of Health business needs, with an emphasis on data management, HIPAA compliance, system security, and technical support. The platform must support client data collection at program intake, discharge, and 6-month follow-up, with capabilities to upload data to SAMHSA's Performance Accountability and Reporting System (SPARS). No specific set-aside designations were mentioned in the solicitation.

Addendum #5 This addendum forms a part of the contract documents and modifies the original drawings and/or specifications for the subject work. In case of conflicts between this addendum and previously issued documents, this addendum shall take precedence.

· Addendum 3, Question 6. The response “only the 4 existing enclaves” is not clear. The building has 5 existing enclaves, 4 of them have an opening of approximately 3’0x7’0 but the 5th enclave has an opening of approximately 6’0x7’0. Can you please confirm if all 5 enclaves require doors? All enclaves (arctic entry ways) will need doors installed

· Addendum 3, Question 5, would the owner like ceiling tile grids replaced as well, or just the tiles? Just the tiles are to be replaced, the grid will not replaced.
