About this file
This is a Cost Proposal Submittal Form for RFP 2026-1600-0063 issued by the Alaska Department of Health's Division of Behavioral Health and the Municipality of Anchorage. The procurement seeks a Software-as-a-Service (SaaS) technology platform to collect client-level data for SAMHSA's Unified Performance Reporting Tool (SUPRT) from State Opioid Response (SOR) subrecipient grantees and automate batch upload of these data to SAMHSA's Performance Accountability and Reporting System (SPARS). The project involves a three-year contract term with deliverables structured across seven categories: security assessment completion, HIPAA-compliant hosting solution, data management, maintenance and licensing, report development, compliance reporting and quality review, and technical assistance. Initial implementation activities during months one and two include completion of a security assessment and customization of the data collection portal and batch upload solution, followed by production launch in month three. Subsequent months involve ongoing data management, batch upload processing, training development, website maintenance, technical assistance, and grantee support. The RFP was re-released on April 24, 2026, with questions due May 4, 2026, and proposals due May 15, 2026.
All proposed costs must be entered into the designated yellow-highlighted cells of the Cost Proposal table for each deliverable and contract year, with the worksheet automatically calculating yearly totals and the total evaluated cost. The total evaluated cost cannot exceed $750,000.00 to be considered responsive. Offerors must provide all-inclusive pricing that accounts for personnel costs including salaries and fringe benefits, direct and indirect costs, materials and supplies, equipment, and administrative and overhead expenses. The Cost Proposal Form presented shows all deliverables currently priced at $0, indicating this is a template form awaiting completion by prospective contractors. The pricing structure requires cost allocation across the three contract years based on the timeline of when work activities are expected to occur, with the Timelines and Activities table serving as guidance for appropriate cost distribution among the seven deliverables.

| Offerors must enter all proposed costs in the yellow-highlighted cells within the Cost Proposal table below. Costs should be provided for each deliverable (DEL.#1–DEL.#7) and for each Contract Year, as applicable. The worksheet will automatically calculate yearly totals and the Total Evaluated Cost. |
The Table 1. Timelines section above is provided to clarify when each associated deliverable is expected to occur. The “Associated Deliverables” column identifies which deliverable numbers correspond to specific activities and time periods (e.g., Months 1–2, Month 3 and Ongoing).
These deliverable numbers directly cross-reference the deliverables listed in the Cost Proposal table below:
For example, activities marked with “3” in the Timelines table correspond to DEL.#3 – Data Management in the Cost Proposal table.
If multiple deliverable numbers are listed (e.g., “3, 4, 7”), those activities relate to multiple cost categories and should be reflected accordingly in the pricing. Total evaluated costs in excess of $750,000.00 referenced in RFP SEC. 1.02 - Budget - will be considered non-responsive.
Offerors should use the Timelines table as a guide to:
• Understand when work will occur, and
