About this file

This document is a past performance reference information sheet template for a Department of the Navy solicitation seeking a Middle Term Availability involving 108 work items to be completed on a ship within the Mediterranean. The template requests contact and pricing information for relevant past contracts, and asks references to describe whether prior work was completed on schedule and within budget and to disclose any quality, delivery, or cost issues. It also solicits summaries of prior work performed as a prime contractor or subcontractor, including ship systems and spaces managed, original equipment manufacturers overseen, vessel type and dimensions, location of work, and use of contractor-owned versus outsourced facilities.
