| Instructions: For the purposes of this table, the Offeror shall provide the following information for this solicitation, including all options. Use tab 1 for the prime and tab 2 etc. for each subcontractor as goals for each subcontractor must be listed separately | |
| ***Note to contract specialist | Update the table headers and footers with pertinent solicitation information. From the toolbar, Select View and then Page Layout from the Workbook Views to insert Header and Footer information. |
Row3 Fill in the Offeror's Name.
Row4 The Total Proposed Contract Value.
Row5 The Total $ amount to be subcontracted to large and small businesses.
Cell B7 The Total $ amount to be subcontracted to large businesses.
Cell B8 The Total $ amount to be subcontracted to small businesses.
Cell B9 The Total $ amount to be subcontracted, where:
Total $ (B9) = Large $ (B7) + Small $ (B8)
Cell C7 The % of Total Contract Value for large businesses, where:
% (C7) = Large $ (B7) / Total $ (Row4)
Cell C8 The % of Total Contract Value for small businesses, where:
% (C8) = Small $ (B8) / Total $ (Row4)
Cell D7 The % of Total amount to be subcontracted to large businesses, where:
% (D7) = Large $ (B7) / Total subcontract $ (Row5)
Cell D8 The % of Total amount to be subcontracted to small businesses, where:
% (D8) = Small $ (B8) / Total subcontract $ (Row5)
Cell D9 The % of Total Amount to be subcontracted to large plus small businesses must equal 100%.
Rows 13-17, Columns B-D For each "Small Business (SB) Concern Subcategory" in Rows 13-17, reference the instructions for Cells B8, C8, and D8, respectively, when providing the "$ to be Sub-Contracted" in Column B, "% of Total Contract Value" in Column C, and "% of Total Amount to be Sub-Contracted" in Column D.
Consider Row 13 as an example:
C13 = B13 / Row 4 and D13 = B13 / Row 5
Note: The total amount of $ to be Sub-Contracted for all small business subcategories will not necessarily equal the total amount to be Sub-Contracted to small businesses.
