About this file

This file is a Quality Assurance Surveillance Plan (QASP) Matrix for isolation transformers under solicitation N3904026Q3133. The QASP establishes performance standards and acceptable quality levels (AQL) for three categories of isolation transformers: twelve 400KVA units capable of 400A output, one 800KVA unit capable of 800A output, and one 1250KVA unit capable of 1200A output. All transformers must convert nominal 460V three-phase ungrounded primary power to 460V three-phase grounded secondary power, with the ability to transform power across the 440-480V three-phase voltage range. The surveillance plan mandates 100% compliance with zero deviations from referenced procedures unless approved by Portsmouth Naval Shipyard (PNSY), with validation conducted through user complaints and customer surveys.

The QASP includes requirements for repair and replacement of defective equipment, specifying that contractor technicians must arrive on-site within 24 hours of notification and complete repairs within 48 hours or provide replacement equipment within 72 hours. The plan employs a five-tier performance rating system ranging from "Exceptional" (exceeds contractual requirements with flawless execution) to "Unsatisfactory" (does not meet most contractual requirements). "Satisfactory" represents the expected baseline performance level where all contractual requirements are met with minor non-conformances and timely corrective actions. Government oversight methodology includes validation of complaints, user surveys, and verification of complete and traceable Operational Quality Evaluations (OQE).

Quality Assurance Surveillance Plan, Matrix
Isolation Transformers SSN 461
Deliverable or Service RequirementPerformance StandardsAcceptable Quality Level (AQL)Method of Surveillance
Meets specified electrical requirements for 400KVATwelve (12) isolation transformer(s) that can take nominal 460V 3 Phase ungrounded power on the primary side and create 460V grounded power on the secondary side. The capacity of each isolation transformer shall be a minimum of 400KVA and provide 400A of power. The transformers shall have the ability to transform 400KVA over the 440-480 3 Phase voltage range.100% compliance. Zero deviations from the referenced procedure unless approved by PNSY.Validated complaints with user/customer and user survey.
Meets specified electrical requirements for 800KVAObtain quantity One (1) isolation transformer(s) that can take nominal 460V 3 Phase ungrounded power on the primary side and create 460V 3 Phase grounded power on the secondary side. The capacity of each isolation transformer shall be a minimum of 800KVA and provide 800AMPs of power. The transformers shall have the ability to transform 800KVA over the 440-480 3 Phase voltage range.100% compliance. Zero deviations from the referenced procedure unless approved by PNSY.Validated complaints with user/customer and user survey.
Meets specified electrical requirements for 1250KVAObtain quantity One (1) isolation transformer(s) that can take nominal 460V 3 Phase ungrounded power on the primary side and create 460V 3 Phase grounded power on the secondary side. The capacity of each isolation transformer shall be a minimum of 1250KVA and provide 1200AMPs of power. The transformers shall have the ability to transform 1250KVA over the 440-480 3 Phase voltage range.100% compliance. Zero deviations from the referenced procedure unless approved by PNSY.Validated complaints with user/customer and user survey.
Defective/Need for Repair EquipmentWithin twenty-four (24) hours of initial notification of the need for repair, the Contractor shall have a knowledgeable and equipped technician on the job-site fully prepared to repair the rental equipment. The defective unit(s) shall be repaired within forty-eight (48) hours of initial notification or, if the original defective unit(s) cannot be made operational, replaced within seventy-two (72) hours. Para 5.2.1100% compliance. OQE must be complete, accurate, and traceable.Validated complaints with user/customer.
RatingsDescriptive Rating System
ExceptionalPerformance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule.
Very GoodPerformance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review.
SatisfactoryPerformance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time.
MarginalPerformance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight.
UnsatisfactoryPerformance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project.
