BLK 4 - Section 2 of DI-MGMT-81945 is tailored as below:

The Contractor shall prepare and deliver to the Government a Phase-Out Plan that facilitates its best efforts and cooperation to effect an orderly and efficient transition to a successor at the expiration of the final task order. The plan shall specify a training program and a date for transferring responsibilities for each division of work described in the plan, and shall be subject to the Government’s approval. The plan shall be submitted not less than 90 days prior to the expiration of the final task order. The plan should include:

· A detailed methodology in logical sequence to ensure a smooth transition of tasks

· A flexible and adaptive approach to accommodate the successor

· Inventory of all services and materials required to fully perform the task order requirements

· Actions required of the Government

BLK 9 - The following information shall be included on the deliverable (Reference: DoD Instruction 5230.24 (28 Apr 2016):

“DISTRIBUTION STATEMENT C: Distribution authorized to U.S. Government agencies and their contractors, Administrative or Operational Use, 12 Apr 2017.”

BLK 12 – No less than 90 days prior to the expiration of the final task order.

BLK 13 - The Government requires 15 calendar days to review. The contractor shall deliver final report 10 calendar days after receipt of Government comments.

BLK 14 - Draft, regular, and reproducible copy shall be delivered electronically (format noted below) and posted to a contractor established and maintained website/integrated data environment. The contractor shall notify the Government of deliverable availability.

16. REMARKS (Continued) Data Item shall be delivered electronically. The format shall be in standard MS Office application and in Adobe Acrobat (as appropriate). Data Item shall be maintained in the electronic format (in an organized ISO-9001 fashion) in an electronic storage area, or on a secured Web page, and submitted through electronic mail, or as directed by the Government representative. If applicable, drawings shall be developed and delivered in Visio/Visionael. In the event of discrepancy between statements in contract references of Block 5 or this CDRL and the Authority in Block 4, contract references and this CDRL govern.
