About this file
This is a pricing proposal template for the GSA Federal Supply Schedule 65 Drugs, Pharmaceuticals & Hematology Related Products contract (M5-Q50A-03-R8). The file is an Excel-based pricing worksheet designed for contractors submitting offers under this multiple award, fixed-price IDIQ contract with Economic Price Adjustment (EPA) provisions. The solicitation covers Federal Supply Schedule FSC Groups 6505 and 6508 for drugs, pharmaceuticals, and hematology-related products supplied to the Department of Veterans Affairs Veterans Health Administration under Veterans Integrated Service Network 12.
This is a standing solicitation issued on an open and continuous basis with no closing date; new offers may be submitted at any time. Contract awards are effective upon award date with a five-year performance period unless canceled, terminated, or extended. The solicitation is a refreshment of the previous M5-Q50A-03-R7 contract. For additional information, contractors should contact the FSS Helpdesk at 708-786-7737 or helpdesk.ammhinFSS@va.gov.

42-2A Dual - BIG 4
| Product Information | MFC Information | Tracking Customer Information | FSS Information | FCP Information | ||
| SIN# | NDC 1 |
Lydia McKay: NDC numbers must follow the prescribed format as follows, entering leading zeroes if necessary:
NDC1 = 5 digits NDC2 = 4 digits
| NDC2 = 2 digits | NDC2 | |||||||||||
| (4 digits only - Add leading 0, if necessary) | NDC3 | Type "YES" if item is handled by Specialty Distrib. | Generic Name / Strength | Trade Name / Strength | Package Size | Unit of Sale | Quantity in Unit of Sale | Unit Pkg | Quantity in Unit Package | Unit of Measure | FDA Application Number | Country of Origin |
(See Country Codes Tab)
| Lydia McKay: You must list the country of origin for each product, utilizing the two-letter country codes provided on a Country Code tab. Remember, the country of origin of the product is where the product was “substantially transformed”, not necessarily the country of manufacture or the country of export. | Total (comm. & gov’t) units sold during Prior Fiscal Year (or a more recent 12-month period) | Total (comm. & gov’t) sales ($) during Prior Fiscal Year (or a more recent 12-month period) | Commercial List Price ($) |
| Lydia McKay: The commercial list price must be listed for each proposed line item. This should match the commercial pricelist/catalog provided with your proposal. The discount off of list price (for your Most Favored Customer, the Tracking Customer, and the Government) will be calculated based upon the information in this column. | MFC Price ($) |
VA Employee: Lowest upfront (guaranteed) discounted price.
| List the lowest price you charge any customer (known as the Most Favored Customer) for each line item. This is specific to each line item; therefore, the Most Favored Customer (MFC) may vary from item to item. In determining your lowest price, you should be looking for the lowest price currently received by any customer for each line item based upon the upfront (guaranteed) discount received. This is not inclusive of “conditional” discounts, such as prompt payment discounts, annual rebates based on reaching a threshold, etc., which must be listed in Fig 515.4-2. | MFC % Off Comm. List Price |
| Christine Szrom: Column auto-calculates the % discount off of list price. | MFC Name |
| VA Employee: Name of Customer or category of customer that receives the lowest upfront (guaranteed) discounted price. | Proposed Tracking Customer (TC) Name |
VA Employee: Please enter the proposed Tracking Customer (TC) even if the proposed TC is the same as the stated MFC.
| List the name of the customer or category of customers you wish to propose as the Tracking Customer (TC). You will need to complete this field for each proposed item. Be prepared to discuss your rationale for the TC you are proposing. The TC ratio (Government price without IFF divided by the TC price) will impact future price increases and decreases under your Government contract pursuant to clauses 552.238-75Price Reductions and 552.216-70 Economic Price Adjustment (located in Document 01 – Solicitation Document). | Proposed TC Price ($) | ||
| Lydia McKay: List the price that the listed Tracking Customer (TC) pays for each line item. If you are proposing different TCs for different line items, then make sure that the price you list corresponds to the TC listed in the previous column. | TC % Off Comm. List Price | ||
| Christine Szrom: Column auto-calculates the % discount off of list price. | Total sales ($) to Proposed TC during Prior Fiscal Year (or a more recent 12-month period) | Total Gov't sales (FSS & open market) ($) during Prior Fiscal Year (or a more recent 12-month period) | Proposed BIG 4 Price ($) without IFF |
| Christine Szrom: List the price you wish to offer BIG4 agencies for each line item. This represents the price without IFF which will be added in Column P. This price must be F.O.B. destination (see clause F-FSS-202-G Delivery Prices in Document 02 – Vendor Response Document). | Proposed BIG 4 % Off Comm. List Price w/out IFF | ||
| Christine Szrom: Column auto-calculates the % off list price using the proposed BIG4 price without IFF. | Proposed BIG 4 Price ($) with IFF |
Christine Szrom: Column autocalculates the price offered to BIG4 including the Industrial Funding Fee (currently 0.5%).
The correct method for calculating the IFF is:
Proposed FSS pricing / (1 minus the applicable IFF) = correct calculated proposed FSS price.
For more information on IFF, see clause 552.238-74 Industrial Funding Fee and Sales Reporting in Document 01 – Solicitation Document.
Lydia McKay: You must list the country of origin for each product, utilizing the two-letter country codes provided on a Country Code tab. Remember, the country of origin of the product is where the product was “substantially transformed”, not necessarily the country of manufacture or the country of export. FCP Price ($) Is Proposed Gov't Price w/out IFF Less Than or Equal to FCP?
