About this file
The document is a Past Performance Questionnaire for the FBI Lakehurst and West Point Operations and Maintenance Services procurement (Solicitation Number: LWP61725). The questionnaire is designed to evaluate a contractor's performance across six key criteria: Quality of Service, Schedule, Cost Control, Business Relations, Management of Key Personnel, and Utilization of Small Business. The rating scale ranges from Exceptional to Unsatisfactory, with references to be completed by previous contract managers and submitted directly to FBI Contracting Officer Maureen Murdock and Contract Specialists Javia Warner and Aaron Awai.
The questionnaire requires detailed project identification, reference contact information, and a comprehensive assessment of the contractor's performance. The final section asks the reference to indicate whether they would have any reservations about soliciting the contractor for future work. The document is part of the FBI's procurement process for obtaining operations and maintenance services at Lakehurst and West Point locations, with the goal of thoroughly vetting potential contractors' past performance and capabilities before contract award.
