About this file
This is a Local Business Entity or Local Impact Certification form issued by the City of Philadelphia as Appendix D to a Request for Proposal (RFP) for The Future Track Program, a professional skills training initiative designed to prepare trainees for careers in residential and business services within Pennsylvania. Applicants may submit this certification to receive favorable consideration in the City's evaluation process by demonstrating compliance with Mayoral Executive Order No. 04-12. The certification offers two pathways for qualification: Local Business Entity status or Local Impact commitment. Applicants must complete the relevant sections and return the form with their application, including a separate statement in their proposal acknowledging their belief that they meet the applicable criteria.
The Local Business Entity certification requires applicants to demonstrate that they filed a Business Income and Receipts Tax return with the City within the preceding 12 months and have maintained a valid Commercial Activity License and all necessary permits for 18 months prior to application. Applicants must also maintain a staffed office within the City and meet at least one of two conditions: either maintain their principal place of business in the City, or satisfy at least two of three requirements including employing more than 60% Philadelphia residents, having more than 50% of full-time employees work in the City at least 60% of the time, or reporting more than 75% of gross receipts as Philadelphia receipts. Alternatively, applicants may pursue Local Impact certification by committing to employ City residents and perform contract work within Philadelphia. The form includes checkboxes for each certification option and requires authorized signature with date and printed name and title of the authorized representative.
