About this file

This document is an RFP Attachment C - Request for Proposal Submittals Checklist for the Thin Sprayed on Liner (TSL) project at Fermi National Accelerator Laboratory. The checklist outlines mandatory requirements for proposal submissions, including specific documentation like pricing lists, proposal certifications, safety certifications, and corporate documentation. Key technical evaluation criteria require bidders to demonstrate: 1) past experience with 3 TSL projects over 5 years valued at $1M each, 2) comprehensive training and certification details, and 3) an Environmental Safety and Health (ESH) corporate program compliant with DOE and OSHA regulations. Additional mandatory requirements include annual revenues exceeding $1 million, a corporate quality assurance plan, an environmental health and safety plan with an Experience Modification Rating (EMR) under 1.0, and a project manager resume.

The solicitation is for a competitive Indefinite Delivery/Indefinite Quantity (IDIQ) contract under NAICS 23832 to apply TSL to shotcrete liner at the SURF facility in Lead, South Dakota. The project aims to test TSL application at the 4850 Level, with up to three IDIQ subcontracts potentially awarded. The test application will occur in the 4850-40 "Spray Chamber" location, with work accessed through the Ross Shaft. The performance period covers lab testing, field trials, and test reporting, with an expected start date of July 2025. Bidders should note work site access limitations, including shaft dimensions and potential work stoppages due to weather, with access available 5 days per week, 24 hours per day.
