About this file
This is a Contractor Conflict of Interest Disclosure Form (L6 level) designed for use in the solicitation HQ003426RE123 for exam delivery and proctoring services.
The form serves as a template to capture conflict of interest disclosures from prospective contractors bidding on the Department of War (DoW) acquisition and financial management functional area certification exam proctoring contract. This L6-level disclosure form is intended to identify potential conflicts of interest that contractors may have regarding their ability to provide impartial, professional exam delivery and proctoring services on a global basis, whether delivered virtually or in-person. As a template document, the form itself does not specify substantive requirements or deliverables but rather establishes the mechanism by which bidders must declare any conflicts, relationships, or circumstances that could compromise their objectivity or independence in administering these professional certification examinations for the DoW acquisition workforce.
