About this file

This is a past performance questionnaire template for Level 5 evaluation criteria related to the Department of War's exam delivery and proctoring solicitation (HQ003426RE123). The questionnaire is designed to assess offerors' relevant experience in providing commercial third-party test proctoring services, specifically their capability to administer professional certification exams for a federal acquisition workforce on a global, on-demand basis through both virtual and in-person modalities.

The questionnaire serves as an evaluation tool to substantiate offerors' qualifications and track record in delivering similar exam proctoring and certification services. Prospective contractors should prepare responses demonstrating their past performance in administering high-stakes professional certification exams, managing distributed test administration environments, ensuring exam security and integrity, and providing customer service to federal or similarly complex clientele. The evaluation criteria at Level 5 likely represent the highest or most critical performance standards, requiring offerors to provide detailed examples, metrics, and references that validate their ability to successfully execute this requirement for the Department of War's acquisition workforce certification program.
