About this file

This is a Past Performance Questionnaire template for solicitation HQ003426RE123, issued by Washington Headquarters Services (WHS) under the Department of Defense (DoD). The questionnaire is designed to collect vendor reputation and past performance information from references familiar with a prospective contractor's work on exam delivery and proctoring services. Responses should be submitted to roberto.saenz2.ctr@mail.mil and Earl.E.Anderson51.civ@mail.mil, and the information provided will remain confidential from the quoter.

The questionnaire requests references to rate contractor performance across six key evaluation areas using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable): customer relations and issue resolution; exam delivery system accessibility and usability for both in-person and virtual formats; flexibility in executing exam publishing and revisions with rapid turnaround; seamless data integration with customer systems; reliable access to examinee data and exam performance metrics; and exam proctoring and delivery system security, reliability, and capacity. The form collects information about the referenced contract including contract number, period of performance, contract value, and contractor role (prime, subcontractor, or team member), along with rater identification and comments supporting the performance ratings.
