About this file

This is a Contractor Performance Assessment Questionnaire template (Attachment L-5) for solicitation N6134025R0004 from the Department of the Navy Naval Air Systems Command. The questionnaire is designed to collect past performance information from offerors' previous contracting officers, administrative contracting officers, program managers, and other points of contact.

The template requires offerors to provide contract administrative data including contract numbers, values, period of performance, and role descriptions. Evaluators must rate contractor performance across three key areas: Quality (conformance to requirements and standards), Management (business orientation and staff management), and Cost Control (forecasting and managing contract costs). Ratings range from Unsatisfactory to Exceptional on a five-level scale. Completed questionnaires must be submitted to the contracting officer and specialist within two weeks of receipt, with specific examples required to substantiate ratings. The document is part of a larger solicitation package for internal Contractor Support Services (iCSS).

Rating System Table:

A common five level assessment rating system is used to evaluate a contractor's performance. Ratings range from Unsatisfactory to Exceptional. Here's a breakdown of each category:

Rating
Definition

Exceptional

Performance meets contractual requirements and exceeds many to the Government's benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good

Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory

Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal

Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.

Unsatisfactory

Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.

1. Given what I know today about the contractor's ability to perform in accordance with this contract or order’s most significant requirements, I WOULD £ or WOULD NOT £ recommend them for similar requirements in the future.

2. Quality: Assess contractor's conformance to the contract/order requirements and standards of good workmanship (e.g., commonly accepted technical, professional, environmental, or safety and health standards. Does the contractor’s delivery and quality of services; to include meeting technical requirements and responsiveness, help to mitigate and/or resolve issues that arose during contract performance? Are reports/data accurate? What degree of Government direction was required to solve problems that arose during performance? Did the contractor maintain an adequate availability of required competent professional service employees?

Provide examples that substantiate rating:

3. Management: Indicate your overall level of satisfaction with the contractor's management (at the local site level, program office, and headquarters). Assess contractor's business orientation (focus on meeting customer needs first, profit first, proactive and aggressive pursuit of problem solving, delays in acting on program needs - a good business partner?). Does the contractor mitigate and/or prevent high work turnover or qualification issues during performance. Is the contractor able to maintain continuity of services with uninterrupted high-quality work? Does the contractor meet end-user needs promptly without demands for contract changes? (This does not refer to work that is clearly out-of-scope). Examples might include changes in hours, inadequate contractor employee performance, and overuse of exemption for level of qualification requests.

Provide examples that substantiate rating:

4. Cost Control: Assess the Contractor’s effectiveness in forecasting, managing, and controlling contract/TO cost. Is the Contractor experiencing cost growth or under-run? (e.g. labor wage rates and indirect rates) If so, discuss the causes and contractor-proposed solutions for the cost overruns. In addition, the extent to which the Contractor demonstrates a sense of cost responsibility through the efficient use of resources in each work effort will be assessed.
