Clear AFS Code, Safety and Environmental Guide
PART 1 GENERAL
1.1 SCOPE
A. This Section covers the code, safety, and environmental requirements applicable to this project.
1.2 REGULATIONS, STANDARDS, AND CODES
A. Construction which is not governed by a Local Building Code or the Contract Specifications shall be governed by the more stringent provisions of the latest published edition or Statute adopted edition of the following applicable Codes, Regulations, and Standards:

1.3 LICENSES AND PERMITS
A. The Contractor shall obtain and maintain current for the duration of this Contract, all required Federal, State and Local licenses and permits. All associated fees and taxes shall be paid by the Contractor without additional cost to the Government. Obtain Open Flame permits from the Military Organization having jurisdiction over the job site prior to any welding or use of open flame. Permits for welding or open flames are valid only for the day of issue.
1. Contractor will advise the Contracting Officer of any requirement(s) for above mentioned permit at least one day in advance of the proposed date of issue. An AF Form 592, USAF Welding, Cutting, and Brazing Permit, will be obtained by the Contractor from the Base Fire Chief prior to any welding operations. Welding shall conform to AFOSH Standard 127-5 and such operations will be inspected and approved by Base Fire Department.
B. Obtain from Base Security all required vehicle and personnel entry permits.
C. Obtain from the Contracting Officer any additional Clear AFS required permits. Current Base permit requirements will be provided to the Contractor at the Preconstruction Conference.
1.4 SAFETY
A. Comply with all Federal and State regulations concerning safety of personnel and equipment. In addition, comply with the applicable provisions of the Corps of Engineer's Manual EM 385-11, "Safety and Health Requirements Manual."
B. The use of safety nets and safety belts with lifelines is mandatory on all pitched roofs more than 6 feet above adjacent areas during re-roofing efforts. This requirement will be strictly enforced.
C. The Contractor shall submit a detailed, job-specific safety plan with specific reference to applicable OSHA requirements and a written description of how all aspects of worker safety on the project will be handled. Table 1 contains an example Table of Contents for a typical comprehensive EH&S Plan.

Section 9: Decontamination Procedures – Decontamination of equipment and personnel exposed to hazardous/toxic materials.
Section 10: Safe work Practices and Site Control- Common sense activities when working in areas where contamination is present or assumed to be present.
Section 11: Emergency Response Plan – To include communication instruments, place of refuge, fire protection, Medical Response Plan, Incident Reporting & Near Miss Reporting, 0peration shut down or work stop authority, Spill or Hazardous Material Release, Emergency Telephone numbers, First Aid and Safety and Health Equipment (e.g. Hearing protection, Hard Hat, Eye protection, Gloves, Etc…) and Nearest Medical Clinic or hospital direction and route.
Please note that the complexity and composition of an EH& S Plan will be directly related to the size and potential hazards of the project.
D. Contractor personnel shall attend a pre-construction safety meeting hosted by Clear Power Plant management to discuss lockout / tag out, boiler operations, emergency action plans and plant alarms.
1.5 FIRE-SAFETY
A. Comply with all fire safety and protection requirements in effect on Clear SFS. Government will brief the Contractor on these requirements at the Preconstruction Conference.
1.6 ENVIRONMENTAL PROTECTION
A. The Contractor shall comply with all applicable Federal, State, and Local laws and regulations covering environmental pollution control and abatement in effect on the date of solicitation and the specific requirements stated elsewhere in the Contractor Specifications.
B. The Contractor shall submit an environmental protection plan to the Contracting Officer, which covers the requirements of all waste disposal and Project site operations. The plan shall include, but not be limited to, the use of on base waste disposal site, the names and locations of off base disposal sites, methods used to safely and legally transport wastes to all disposal sites, methods used to prevent non-point source pollution at the Project Site (including a contingency plan for effectively handling spills of hazardous materials), and training for employees on all elements of this plan. No site work, including mobilization, staging and site preparation, shall be permitted prior to approval of the plan by the Contracting Officer. Final work plan approval is contingent upon Contractor meeting all the requirements contained in this section - 1.6 A thru E. Non-compliance with the submittal of hazardous materials utilization information as described in E 2 and 3 of this section can negatively impact project initiation.
C. All construction wastes shall be legally disposed of off- site by the Contractor.
D. All hazardous construction wastes as defined in 40 CFR, Part 261, shall be collected and disposed of in accordance with 40 CFR, Parts 260-265. The Contractor shall not store hazardous wastes on base for more than thirty (30) days.
E. The contractor assumes the reporting requirements listed in this section. Figures offer sample formats for meeting the reporting requirements provided in this section. In addition to reporting to the

Contracting Officer, the applicable reports are due to Environmental Department (ENV) monthly or at contract closeout, whichever occurs first, unless otherwise indicated in the subsection. Information can be submitted by fax at (907) 585-6565, by e-mail at james.ward.70.ctr@spaceforce.mil, or in person to Room 2008 in Bldg. 201.
1. Copies of any required communications with ADEC or EPA must be submitted to ENV for review and retention in the department files. Regulatory communication examples could include APDES NOI and asbestos removal notification.
2. At least 30 days prior to construction contractors must submit to ENV a list (Utilization Request or equivalent) of all hazardous chemicals/materials (HM) to be utilized. The list (Figure 1) should include name of the chemical or commercial product name, quantities, storage location, process description and a Safety Data Sheet (SDS). Subsequent, use of hazardous materials not included in the original work plan must be cleared through Clear’s Environmental department. All required documentation must be submitted prior to approving the material for site entry and utilization.
3. Quantities of HM utilized should be reported periodically or at the conclusion of the project whichever is agreed to by the contract representative(s). The contractor will submit a final HM inventory reporting quantities used and duration on site. In addition, all unused chemical / hazardous materials must be removed from Clear Space Force Station. 4. The contractor must report all off-site diversion and disposal of solid waste required during the completion of the project (Figure 2). This information should be reported as pounds of solid waste and indicated proportionally as municipal solid waste and construction and demolition debris. If it is not possible to report this information in pounds, the volume of waste and the composition of each volume of waste will be provided. Due to AF waste disposal reporting metrics all solid waste disposal information must be reported within 7 days the end of every month.
5. The contractor will report to the ENV office all hazardous waste generated as a result of the project as they occur to ensure proper accumulation procedures are followed. A receipt proving proper off-site disposal at a Solid Waste Landfill must be submitted or mailed directly to the Environmental Department.
6. Prior to beginning asbestos or lead paint abatement the contractor must submit an activities record (Figure 3). The activities record should include the names of the personnel performing the work, supervisor’s name, date(s) of work, description of what is to be abated, quantity and the special safety conditions taken.
7. Submit certification records of all personnel working with hazardous chemicals or abating toxic materials (asbestos & lead). Resume and certification of the abatement supervisor, responsible party or third party IH subcontractor must be included. The certifications can be included as an appendix to the EH& S or the Environmental Protection Plan.
8. Analytical results from required air monitoring in areas where hazardous material PELs or action levels may be exceeded, or sampling activities for identifying asbestos containing materials (ACM) or lead based paint must be submitted to the ENV office.
9. Any hazardous and non-hazardous wastes recycled, reused, or otherwise diverted from disposal will be reported in pounds or as a volume as previously described (Figure 2).
