About this file
This is a Past Performance Questionnaire template used to evaluate contractor performance for a Dorm Construction Contract in Building 2424 at Joint Base McGuire-Dix-Lakehurst, New Jersey. The questionnaire is designated as Source Selection Sensitive information in accordance with FAR regulations.
The template contains four main sections: Contract Identification, Customer/Agency Identification, Respondent Identification, and Performance Information. The Performance Information section uses a 5-level rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) to evaluate 22 specific performance areas including: technical and management capabilities, quality control, schedule adherence, problem resolution, subcontractor management, and small business goals compliance. The questionnaire concludes with questions about any cure notices issued and whether the evaluator would award future contracts to the contractor. Each rating requires supporting narrative information to justify the assessment.
