About this file

The document is an Infection Control Risk Assessment (ICRA) Permit template used to assess and mitigate infection risks during construction or renovation projects in healthcare settings. The template provides a comprehensive matrix categorizing risk levels (Low, Medium, High, Highest) and project types (Type A, B, C, D) with corresponding infection control requirements and mitigation strategies across four risk classes (Class I-IV).

The form details specific protocols for each risk class, including dust control, HVAC system isolation, critical barrier implementation, negative air pressure maintenance, personnel protective measures, and post-construction cleaning procedures. Key requirements include minimizing dust generation, using HEPA-filtered equipment, sealing work areas, requiring shoe covers and protective clothing, maintaining negative air pressure, and ensuring thorough environmental cleaning before removing construction barriers. The matrix helps healthcare facilities systematically manage infection risks during construction and renovation activities by providing a structured approach to infection control based on the specific type and risk level of the project.
