About this file
This is a Contractor Staffing Access Questionnaire used by the Federal Aviation Administration (FAA) to identify the access requirements and security clearance needs of contractor employees in the event of contract award.
The questionnaire requires contractors to provide the total estimated number of employees (full-time, part-time, and subcontractor personnel) who will support the contract throughout its duration, with an option to attach a detailed personnel list. For each labor category, contractors must identify the position title(s), number of employees, and specify whether employees require: (1) routine access to FAA facilities (more than one day per week) for more than 180 days; (2) access to FAA computer systems or networks; or (3) access to FAA Sensitive Unclassified Information (SUI) such as building designs, drawings, research and development materials, and operations plans. The questionnaire provides a template for up to three labor categories with yes/no checkboxes for each access type, and contractors may add additional categories as needed. The information collected is used by the FAA to determine appropriate investigation requirements and badging credentials for contractor personnel.
