About this file

This is the FAA Form 4400-48, a Contractor Staffing Access Questionnaire required by the Federal Aviation Administration to identify access needs of potential contractor employees prior to contract award.

The form collects information regarding estimated total employee count (full-time, part-time, and subcontractor personnel) anticipated throughout the contract period, with provisions to attach proposed personnel lists if available. For each labor category (up to three categories shown on the form), contractors must identify position titles, estimated employee counts, and specify access requirements across three dimensions: (1) routine facility access to FAA facilities for more than 180 days annually, defined as more than one day per week; (2) access to FAA computer systems or networks; and (3) access to FAA Sensitive Unclassified Information (SUI) including building or land designs, drawings, research and development material, and operations plans. Each access category requires a yes/no response. The questionnaire is designed to enable the FAA to determine appropriate background investigation requirements and badging procedures upon contract award. Respondents have approximately 25 minutes to complete the form, which carries OMB Control Number 2120-0595.
