About this file

This is a Contractor Staffing Access Questionnaire (FAA Form 4400-48) used by the Federal Aviation Administration to determine investigation requirements and badging needs for contractor employees.

The form requires contractors to provide estimated total employee counts (full-time, part-time, and subcontractor personnel) anticipated throughout the contract life. For each labor category, contractors must identify position titles and estimated employee numbers, then answer three yes/no questions regarding: (1) whether employees require routine access (more than one day per week) to FAA facilities for more than 180 days; (2) whether employees need access to FAA computer systems or networks; and (3) whether employees require access to FAA Sensitive Unclassified Information (SUI) such as building designs, drawings, research and development materials, or operations plans. The form accommodates up to three labor categories and allows contractors to use FAA-specified categories or their own contractor-defined categories. An optional personnel list may be attached. The burden estimate for completing this questionnaire is approximately 25 minutes per response.
