About this file

This is a Past Performance Questionnaire used by the Military Sealift Command (MSC) to collect reference information on contractor performance. The questionnaire serves as Attachment J-13 to the USNS LUCY STONE (T-AO 209) Post Shakedown Availability (PSA) solicitation (N3220526R6052) and requests responses by June 24, 2024, at 5:00 PM EST to be submitted to contracting officers Doug Jones (Jeffrey.d.jones156.civ@us.navy.mil) and Christopher Ward (christopher.s.ward1.civ@us.navy.mil).

The questionnaire requests evaluations of a referenced contractor's past performance across four specific areas: quality of product or service; timeliness or scheduling of deliveries; business relations and customer satisfaction; and key personnel and staffing including subcontractors. Respondents must rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) with detailed definitions provided for each rating level. The form also requires respondents to provide an overall performance rating, submit comments for Exceptional or Unsatisfactory ratings, and indicate whether they would rehire the contractor for future work. This is a template document designed to solicit objective feedback from past customers to support the government's evaluation of contractor performance history.
