About this file
This is an Insurance Schedule document issued by the State of Nevada outlining mandatory insurance requirements for contractors and subcontractors performing work under state contracts. The document establishes comprehensive insurance coverage requirements that must be maintained throughout the duration of contract performance and any applicable warranty periods. Contractors are required to procure Commercial General Liability insurance with a general aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000, Automobile Liability coverage with a combined single limit of $1,000,000 for all owned, hired, and non-owned vehicles, and Workers' Compensation insurance at statutory levels with Employers' Liability coverage of $100,000 per accident and $500,000 policy limit for disease. All insurance policies must name the State of Nevada as an additional insured and include a waiver of subrogation in workers' compensation coverage.
Contractors must obtain insurance from carriers licensed in Nevada with an A.M. Best rating of not less than A- VII and must provide certificates of insurance on ACORD forms or equivalent before work commences. Notice of cancellation, suspension, or modification of coverage must be provided to the Department of Employment, Training and Rehabilitation, Financial Management Contracts Unit at 500 E. Third Street, Carson City, NV 89713-0001, with thirty days' advance notice (or ten days for non-payment of premium). The contractor's insurance must be primary and non-contributory to all other available sources, and failure to maintain required coverage or provide evidence of renewal constitutes material breach of contract. Any modifications to insurance requirements must be approved by the Attorney General's Office or Risk Manager, and subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates of insurance meeting the same minimum requirements.
