About this file

This is an Invitation for Bid (IFB) Offer form submitted to the State of Illinois for JPMC Attenuator, Crash Cushion and Parts under solicitation B-49930. The document serves as the bidder's formal offer to provide the specified products in compliance with the solicitation requirements. Bidders are required to complete all sections of the form, including documenting required submissions as outlined in solicitation document section A.1, with failure to meet requirements resulting in disqualification. The form provides space for bidders to detail items that qualify for any claimed preferences and to submit exceptions to standard contract terms and conditions, with the State retaining the right to accept or reject any exceptions proposed.

The State of Illinois offers various business preferences under the Illinois Procurement Code, including the Illinois Business Preference (requiring completion of an affidavit) and the Domestic Product Preference (requiring designation in the Alternate Description field for qualifying line items). All resulting contracts will be governed by Standard Terms and Conditions available on the CPO General Illinois website. Bidders must submit redline versions of the Standard Terms and Conditions if taking exception to any contract terms. The form includes checkboxes for various preferences and fields for bidder signature, authorization, and submission date, though the specific pricing, quantities, delivery schedules, and other substantive contract details are not populated in this version of the offer form.
