The document requires the Apparent Low Bidder to complete and submit this form by 4:00 p.m. local time on the fifth full business day after bid opening, as part of the city's process to ensure contractors meet required standards for contract award. The form seeks comprehensive information about the contractor's performance history, legal standing, and capability to complete the project.
The document requires the contractor to disclose critical information including whether they have been debarred, suspended, or convicted of certain offenses in the past three years, any history of liquidated damages, contract terminations, legal claims, or bond issues. Additionally, the contractor must provide their current Experience Modification (EMod) Rating, describe three representative similar projects with detailed project information, list current workload and resource availability, and certify their ability to perform the project without anticipated legal or bankruptcy challenges. The form emphasizes that failure to provide sufficient information may result in being found non-responsible and potentially disqualified from the bidding process.
