Description/Specifications/Statement of Work

PERFORMANCE WORK STATEMENT

1.0 GENERALINFORMATION

1.1 This is a non-personal services contract to provide Water Sample Collection and Analysis Services for Commercial & Residential Sectors.

1.2 Description of services/introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, and supervision necessary to perform water sample collection and analysis services as defined in this Performance Work Statement (PWS). The water sample collection and analysis shall be performed at Shaw Air Force Base(AFB), Poinsett Range, and Wateree Air Force Recreational area. The contractor shall collect, label, transport, and analyze water samples from various sources including ground water sources (e.g., wells), surface water bodies (e.g., lakes, rivers), and points within water distribution systems. The contractor must assess water quality, determine compliance with relevant regulations, and provide recommendations for remediation or treatment, if necessary.

1.3 Background: The 20th Medical Group (MDG)-Shaw AFB Bioenvironmental Engineering (BE) services include indoor air quality surveys, drinking water surveillance, and occupational health risk assessments. The BE is acquiring the water sample collection and analysis services for Shaw AFB, Poinsett Range, and Wateree Air Force Recreational area via contract No.FA480323C0006.

1.4 Scope:

1.4.1 The contractor shall provide the required certified reference materials, equipment, qualified personnel, documentation, and transportation needed to perform the water sample collection and analysis services. The contractor must provide water sample collection and analysis services for Shaw AFB, Poinsett Range, and Wateree Air Force Recreational area utilizing Environmental Protection Agency (EPA) approved analytical methods. The contractor shall comply with the quality control requirements per method. The bacteriological and chemical analysis services at these commercial and residential areas include collecting and analyzing water samples from various sources such as wells, distribution systems, and lakes, rivers, swimming pools, assessing water quality and safety, identifying contaminants, monitoring environmental conditions, determining regulatory compliance, and recommending solutions for remediation or treatment options.

1.4.2 The contractor shall analyze samples collected by the 20th MDG-Shaw AFB BE. The contractor shall provide required sample container(s) and documentation materials per method, preservation requirements per method, trip blanks, matrix spikes, and transportation of container to Shaw AFB and samples for analysis from Shaw AFB or sample collection location. The contractor shall coordinate with the BE to schedule delivery of sample collection materials, sample collection, handling, and analysis per method(s) requirements within 24 hours of request.

1.4.3 The contractor must perform the following analytical methods per year. The quantities are estimates and may vary throughout the life of the contract. The listed methods may be updated pursuant EPA new and emerging contaminant or improvement on analysis throughout the life of the contract. It is critical that sampling is performed with enough lead time each month to ensure all reports are submitted to BE not later than the 4th of next month.

1.4.3.1. Frequency: The contractor must travel two (2) times per month to Shaw AFB. The following sample collection and analysis frequency is an estimate and may be subject to change:

1.4.3.1.1. Method 533: three (3) monthly and one (1) every three (3) months.

1.4.3.1.2. Method 9223B Colilert; 4500-H+B; and 4500-CI G: Six (6) monthly divided into four (4) on the first trip and two (2) on the second trip.

1.4.3.1.3. Method 200.8: The Sample collection is performed by BE. The contractor must coordinate with BE the drop off of the sample collection materials and sample pick-up during the two (2) monthly trips to Shaw AFB.

1.4.4 Analysis reports and documents: The contractor must provide sample reports as identified in the Deliverables Table in Exhibit 1. The reports shall include official compliance forms and chain-of-custody documentation. These reports must be signed by both the chemist who performed the analysis and the laboratory director who reviewed the results. All reports shall provide clear and concise reporting of analytical results and must contain the following components at minimum:

1) Title of report (i.e., test report, test certificate)

2) Analytical methods used.

3) Sample identification and description, sample location, and date/time of sample.

4) Analytical parameters tested.

5) Analytical results (including units of measurement).

6) Identification of individual taking the sample.

7) Quality Control (QC)/Quality Assurance (QA) data.

8) Dilution factors as applicable.

9) Reportable detection/regulatory limits, Method Detection Limits (MDLs) and data qualifiers definition.

10) Chain-of-custody for each sample.

11) Field records for analysis required in the field (i.e. pH and free available chlorine results).

12) Analytical data from network partners, if applicable, with same criteria for sample results.

13) Laboratory Certification Information.

1.4.5 Emergencies Or Special Events: On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours. The MTF will submit Emergencies or special events requests for any of the three locations - Poinsett Range, and Wateree Air Force Recreational area during the contract period of performance as necessary. The MTF will provide the method and location with their request to the contractor when testing is needed.

Request for emergencies or special events can also occur when Analysis results are noncompliant for any of the methods. The contractor shall notify BE Point of Contact (POC) for noncompliant samples immediately after the incubation period and coordinate with BE for additional sample collection within 24 hours of notification. Analysis results must be delivered promptly based on the method.

1.4.6. The contractor may be required to perform the following analytical methods in emergencies or special events. The quantities are estimates and may vary throughout the life of the contract. 1.4.7 Valid Sampling/Analysis: for any laboratory actions or lack thereof, that compromise the validity of the sample, the contractor shall provide a repeat sample collection and analysis at no charge to the Government. The Contractor shall invoice the customer only after valid sample results are reported.

1.4.8 The contractor shall strictly adhere to Federal Occupational Safety and Health Agency (OSHA) Regulations, EPA Regulations, and all applicable federal, state, and local regulations including but not limited to the applicable publications, clauses, provisions, and deviations listed herein.

1.4.9 Sampling and Analysis Plan (SAP): The contractor shall provide a comprehensive SAP tailored to the analysis methods outlined in this PWS. The SAP shall describe the sampling locations, numbers and types of samples to be collected, the quality control, holding time, and handling requirements per method. Including clear instructions on field sampling preparations, collecting process, procedures, handling, and transportation details of samples for each analysis method listed in this PWS; analysis request submission process; contact information for sample tracking and technical inquiries, mailing instructions, and emergency contact information.

1.5 Period of Performance (PoP):

1.6 Administrative specifications

1.6.1 Place of performance: The work shall be performed at 20th Medical Group, 420 Polifka Drive, Shaw AFB, South Carolina 29152; Poinsett Range: Range Rd Sumter, SC 29154; and the Wateree Air Force Recreational Area: 2030 Baron Dekalb Rd, Camden, SC 29020.

1.6.3 Service Contract Labor Standards Wage Determination(WD): The Services Contract Labor Standards Act applies to this contract. WD No.: 2015-4439. Note: The most recent Wage Determination (WD) revision available at time of award will be added to the resulting contract award.

1.6.4 Per FAR 52.222-43 Fair Labor Standards Act and Service Contract Labor Standards-Price Adjustment; Service Contract Labor Standards Wage Determination Adjustment Request for Unit based contracts: The contractor must provide a detailed labor cost breakdown, including job classifications, anonymized employee information (wage rates, fringe benefits), and a clear explanation of how labor costs are allocated to each water sample collection and analysis service unit, including the average time to perform each service unit and allocation of indirect labor. Supporting documentation must include a copy of payroll records, time study data (if available), and fringe benefit documentation. Finally, the request must clearly calculate the total price adjustment, based on the increase in labor cost per service unit, multiplied by the estimated number of service units to be completed during the contract period, explaining precisely how the adjustment was calculated, demonstrating the direct impact of the wage determination change on the unit-based costs. Adjusted unit price shall not include more than two (2) decimals.

1.6.5 Recognized Federal holidays: Services shall not be required on Federal holidays or the day of observance of federally established holidays and local base holidays (i.e. Easter). In general, federal holidays falling on a Saturday are observed on the preceding Friday whereas holidays falling on a Sunday are observed on the following Monday.

1.6.6 Hours of operation: Shaw AFB hours of operations are from 0730 to 1630 (Eastern Standard Time) Monday through Friday. The contractor is responsible for conducting business Monday through Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.

1.6.7 Base Access: The contractor must coordinate with the COR and BE POC to obtain a visitor's pass and escort while on base property.

Documentation shall be submitted for base passes or base identification per base security requirements. Contractor personnel shall maintain base passes on their person at all times in performance of this contract. Contractor personnel who have personal DoD retiree or dependent ID cards shall not use those personal ID cards for base access when performing contract work. The contractor shall maintain a current listing of employees under this contract with employees' personal information as required by the base security office (i.e., name, social security number, driver license number).

1.6.8 Traffic Laws: The Contractor and its employees shall comply with Shaw AFB traffic regulation. All Contractor personnel who operate a vehicle in performance of this contract shall possess a valid driver's license and current registration and insurance.

1.7 Contractor Identification

1.7.1 Contractor personnel performing services in a contractor capacity in a Government facility are required to possess and wear an identification badge that displays his or her name and the name of their company. Ensure all those with whom the person interacts in any face-to-face dealings while supporting the Government understand that the person is contractor support personnel.

1.7.2 Contractor Representative: The contractor shall submit the contractor representative contact information and any other key personnel, in writing, to the COR, BE POC, and KO. The contractor must provide notification in writing of changes in the status (i.e., termination or replacement) of designated contractor's representative as identified in Exhibit 1.

2.0 SPECIFIC TASKS

2.1 Sample Collection and Analysis: The contractor must collect and analyze water samples according to EPA methods and procedures necessary to prepare sample(s) for analysis.

2.1.1 The contractor must provide instrumentation, professional personnel, calibrated field analysis equipment, safety supplies(e.g., gloves, eye protection), packaging, and transportation needed to perform sample collection and analysis of the specified methods.

2.1.2 The contractor shall adhere to quality controls (e.g., matrix spikes, reagent blanks), handling requirements (e.g., any preservation requirements, sample container requirements, etc.), storage, and holding time per analytical method specifications.

2.1.3 The contractor shall deliver, ship, or pick up samples ensuring that holding times are met for the laboratory to process the sample(s).

The contractor shall immediately report all samples lost or broken in transportation with details of incident.

2.1.4 Sample Labeling: The contractor shall label the samples for identification. The label must include at minimum, sample name, sample location/site, any preservatives added, and date/time of sample collection.

2.1.5 The contractor shall complete chain of custody forms and any applicable sample collection forms at the time of sample collection by the contractor.

2.1.6 Validity Sampling/Analysis(PWS bullet 1.4.7): the contractor shall notify the BE POC immediately following the incident of compromised samples and repeat sample collection and analysis at no charge to the Government.

2.1.7 Sample Collection Schedule: The contractor shall coordinate a sampling schedule with the BE POC for each and all analysis at the three

(3) location areas for the contract period of performance. The contractor shall provide the schedule in writing to the COR withing 15 business days of award. Changes in the schedule shall be communicated and addressed by both parties-contractor and BEPOC at least 24 hours before the scheduled date and time.

2.1.8 The contractor shall provide an SAP in accordance with bullet 1.4.9 of this PWS. The SAP must be submitted to the Contracting Officer (KO), Contracting Officer Representative (COR), and BEPOC within 15 business days of contract award. Changed or updated SAP documents must be provided in a timely manner for sample collection process compliance.

2.1.9 Analysis Reports and documents (PWS bullet 1.4.5): All documentation shall be prepared in accordance with standard industry practices, ensuring electronically produced documents which reflect logical flow of material, tables of contents indexes and page numbering.

Where applicable, the contractor's attention is called to the availability of commercial, industry, federal, and military guides, instructions, and standards.

2.1.9.1 Analysis Results: The contractor must submit via email the reports within five (5) working days of the analysis. The contractor shall provide sample reports including compliance sample forms and chain of custody forms.\

2.1.9.2 Noncompliant results: The contractor shall notify the BE POC by telephone of any non-compliant results as soon as the incubation period is complete. (The BE POC information will be provided with the contract award).

2.1.9.3 Inspection and Acceptance: The BE (or their designee) is responsible for inspection and acceptance of analysis reports and documents withing five working days of receiving there ports and documents.

2.1.10 The contractor must coordinate with BE for additional sample collection and analysis to address emergencies or special events. Analysis results must be delivered promptly based on the method. This shall include recommended solutions for remediation or treatment options.

2.2 Schedule for samples to be collected by the 20th MDG-Shaw AFB BE (PWS Bullet 1.4.2): The BE POC must request and coordinate with the contractor the sample collection process and procedures per analysis method when the sampling schedule is not fixed or definite in the Sample Collection Schedule.

2.2.1 Sample supplies: The contractor shall provide the sampling equipment, preservatives, and procedures, for samples to be collected by the 20th MDG-Shaw AFB BE at least 24 hours before the sample collection date and time. The contractor shall adhere to requirements for types of sample containers, and sample preservation as dictated by the analytical methods, regulatory guidance, and other established guidance.

2.2.2 The contractor shall coordinate sample Pick up location, date, and time. The contractor shall ad hereto requirements for holding times as dictated by the analytical methods.

2.2.3 When the contractor or their courier picks up samples collected by the 20th MDG-Shaw AFB BE, the BE POC (or their designee) will sign and date the pertinent form and documentation to confirm the sample transfer to the contractor, and the contractor/courier will also sign and date the form to acknowledge receipt. If a courier is used, a contractor representative must document the transfer from the courier to the contractor's facility, noting the sample's condition including temperature. The contractor is required to immediately notify the BE in writing if any samples are lost or broken during transport and must also report the condition (including temperature) of the samples upon receipt. If the samples' condition falls outside acceptable criteria, the contractor must notify BE immediately.

2.2.4 Analysis report: the requirements for the analysis reports in bullets 2.1.9.1 and 2.1.9.2 are applicable to analysis performed for samples collected by the BE. In addition, the report shall include a copy of the pickup order form and shipping/delivery documentation.

3.0 Qualifications/Considerations

3.1 Qualifications/Considerations:

3.1.1 The contractor shall submit proof of laboratory certification by the State of South Carolina. The contractor shall maintain and provide current proof of annual certification by the State of South Carolina to the KO and the BE with the initial proposal and prior to execution of any option period. The contractor shall immediately notify KO and BE if this certification is subject to expiration or termination.

3.1.2 The contractor is responsible for ensuring sufficient qualified personnel are assigned to this contract. All personnel must be qualified and certified to perform the requirements listed herein, including those qualifications and certifications (as required) by any original equipment manufacturer (OEM) requirements to maintain, install, or operate equipment for performance of the services required under this contract.

Violations of qualifications and certifications may result in a termination of the Contract.
